This was due to the discovery that a maintenance surveillance test (MST-I0380) on the monitor was not completed as scheduled and was overdue on December 27, 1987.Because of the missed surveillance test, the monitor should have been declared inoperable on December 27, 1987;therefore, the plant must assume that the monitor has been inoperable since that time.Since the monitor was not restored to service within 7 days of that date, a special report is required in accordance with Technical Specification 3.3.3.6, Accident Monitoring Instrumentation.

Test, was successfully completed on January 21, 1988, and the monitor was declared operable on January 26, 1988, at.1655.The cause of the overdue surveillance was a procedural deficiency.

o s o o o 400 8 8 002 0 102 OF The WRGM glWV-3547-1 was declared inoperable on November 2, 1987, due to the potential loss of sample flow during calibration of the stack flow meter and iso-kinetic sampling skid.This inoperability was reported to the NRC in a special report dated January 4, 1988.The routine maintenance surveillance test, MST-I0380, was scheduled to be performed on December 23, 1987.Since the monitor was still inoperable at this time, the maintenance technicians recorded an inoperable status on the surveillance task sheet.When this information was entered into the computerized Surveillance Test Scheduling System (STSS), the task was automatically rescheduled for March 5, 1988, and indicated that the equipment was inoperable.
The system did not indicate that the surveillance test would be overdue on December 27, 1987.However, the monitor was declared operable by Operations personnel on the same day (Dec'ember 23, 1987), without the maintenance staff being aware of this.The Operations staff based their decision to declare the monitor operable on verbal communications from the Radiation Control staff that the monitor had been placed in operation.
Personnel from the Radiation Control staff had been verbally assured by maintenance personnel that there were no outstanding surveillance tasks to be performed on the monitor.As a result, MST-I0380 was not performed and became overdue on December 27, 1987.On January 19, 1988, the plant wa's operating in Mode 1 at 100X power.At approximately 1530 on this date, the Maintenance Supervisor discovered that the maintenance surveillance test (MST-I0380) was not completed as scheduled on the Vent Stack 5A wide range gas monitor (WRGM)glWV-3547-1 (EIIS:IL), and was overdue on December 27, 1987.Operations personnel were notified, and at 1600, the monitor was declared inoperable in accordance with Technical Specification 3.3.3.11, Radioactive Gaseous Effluent Monitoring Instrumentation, and Technical Specification 3.3.3.6, Accident Monitoring Instrumentation.
Because of the missed surveillance test, the monitor should have been declared inoperable on December 27, 1987, the date the surveillance test was overdue;therefore, the plant must assume that the monitor has been inoperable since that time.Since the monitor was not restored to service within 7 days of that date, a special report is required in accordance with Technical Specification 3.3.3.6, Accident Monitoring Instrumentation.

PLP-103, Surveillance and Periodic Test Program does not adequately address actions required to be taken when a routine surveillance test is delayed due to inoperative equipment.
As a'esult, the monitor was declared operable without operations being aware that'he surveillance test would be overdue in 4 days.ANALYSIS OF EVENT: The event was reportable due to noncompliance with Technical Specification 3.3.3.11 and 3.3.3.6.The compensatory actions required by Technical Specification 3.3.3.11 were taken on January 19, 1987, when the monitor was declared inoperable.
Since the monitor was technically inoperable, due to the missed surveillance test, since December 27, 1987, and this was not discovered until January 19, 1988, there were no compensatory actions during this time period.MST-I0380, which was not performed as scheduled, was a digital channel operational test designed to exercise the digital computer hardware, using data base manipulation, to verify operability of the alarm and trip functions as specified in Technical Specifications Table 4.3-9.The monitor was still operating and satisfactorily passed all the routine operational surveillance tests conducted twice daily by Operations personnel in accordance with OST-2044, Radwaste Daily Operations Surveillance Test.Although technically inoperable, the monitor was still functioning and is assumed to have accurately monitored the radiological effluents from vent stack 5A during the period in question.Another event concerning the inoperability of this radiation monitor was reported in a special report dated January 4, 1988, and a Licensee Event Report, LER 87-064-00, was submitted on January 18, 1988, due to failure to meet the time requirements for issuing the special report.CORRECTIVE ACTIONS: 1.The missed surveillance test was completed, and the monitor was declared operable on January 26, 1988.2.To ensure that the Control Room personnel are aware of inoperable-equipment and that an Equipment Inoperable Record (EIR)is generated which correctly reflects overdue or incomplete routine surveillances on a component, PLP-103, Surveillance and Periodic Test Program, will be revised to include the following specific guidance: NRC SORM 344A 1883)R U.S.GPO.1088.0-824 d38/4dd NRC Pons 3$SA)84)3)LICENSEE EVENT REPORT (LERI TEXT CONTINUATION U.S.NUCLEAR REGULATORY COMMISSION APPROVED OMS NO.3IEOW)04 EXPIRES: 8/3l/88 PACILITY NAME ill DOCKET NUMSER l2)LER NUMSER (8)PACE LT)Shearon Harris Nuclear Power Plant Unit 1 TEXT/4 mom<<oco/f ffqv)snf, Moo//)//sao///RC
