SPECIAL REPORT SR-85-001-00 INTRODUCTION

'At 1550 hours0.0179 days <br />0.431 hours <br />0.00256 weeks <br />5.89775e-4 months <br /> on September 5, 1985 Waterford 3 Steam Electric Station was in mode 5 (cold shutdown) when plant personnel, while performing the surveillance requirements specified in Technical Specification 4.3.3.3.1, discovered that the two (2) Triaxial Peak Accelerographs located within the Reactor Building, were damaged due to excessive heat and vibration.

Since the plant conditions needed to produce such damage have not existed at Waterford 3 since mid-July (on July 18 Waterford 3 was removed from service due to high vibrations on the Main Turbine), it is felt that the above accelerographs have been inoperable for longer than the 30 days delineated in Technical Specification 3.3.3.3.a.

(cold shutdown) when plant personnel, while performing the surveillance requirements specified in Technical Specification 4.3.3.3.1, discovered that the two (2) Triaxini Peak Accelerographs (SM-IYR-6020 and -6021) located within the Reactor Building, were damaged due to excessive heat and vibration.

An investigation revealed that due to the excessive heat and vibration associated with the location of each accelerograph, components within tho instruments were melted and damaged rendering the equipment inoperable.

Since these seismic instruments perform only a recording function. it is believed that the inoperability of the above instruments do not pose a safety cencern.

  • i CORRECTIVE ACTION An engineering evaluation has concluded that the present locations of the eccelerographs are not suitable for the equipment design. Therefore, engineering personnel are studying the possibility of relocating the instruments in a more hospitable location. Every effort will be made to restore the instruments to an operable status as soon as practical, however, since the instruments are located in the Reactor Building, it is estimated that the work will not be completed until the end of the first refueling outage.
