Neutron Source DESCRIPTION:
Americium oxide powder mixed with beryllium powder is contained within two concentric stainless steel capsules with each capsule sealed by continuous welds.
Minimum wall thickness is 0.030 inch (0.76 mm).
The outer capsule contains a 1/4-20 NC threaded hole in one end.
The dimensions of the source vary with the amount of contained americium-241 from 1 inch (25. 4 mm) in diameter by 1.32 inch (33. 5 mm) in height, for a 1.0 curie (37 GBq) source, to 1.5 inches (38.1 mm) in diameter by 3.65 inches (92.7 mm) in i
height, for a 10.0 curie (370 GBq) source.

Each source is stamped with 0.125 inch (3.2 mm) letters on the I
end with the model number:
"PL-AmBe- (a number) ".

1 CONDITIONS OF NORMAL USE:
The sources were designed for general use, one of which was oil well logging.
However, the sources are not currently approved for use in well logging applications (reference November 1, 1991 letter from John Glenn, NRC).
The maximum recommended continuous use temperature is 300 C (572 F).
PROTOTYPE TESTING:
The manufacturer had prototype tested the source at 500 C (932 F) for a period of one week and pressure of 5000 psi (34. 5 MPa).

OUALITY ASSURANCE AND CONTROL:
The manufacturer stated that each source was:
1.
Inspected for weld defects.
2.
Pressure tested with 100 psi (689.5 kPa) helium followed by a bubble test in a liquid.
3.
Tested for external contamination with a maximum permissible level of 20 DPM.
4.
Stored for one week and again checked as in 3 for any increase in contamination.
LIMITATIONS AND/OR OTHER CONSIDERATIONS OF USE:
- The source shall only be used by specific licensees of NRC or Agreement States.
- The source shall be leak tested at intervals not to exceed 6 months using techniques capable of detecting 0.005 microcurie (185 Bq) of removable contamination.
Handling, storage, use, transfer, and disposal: To be determined by the licensing authority.

their former facilities were to be investigated further. Due to the lack of documentation in the General Nuclear, Inc. and Parkwell Laboratories, Inc.

In another letter to the Commission dated December 10, h
1969, the licensee stated.that'all radioactive materials were if transferred to their Texas License No. 4-734 and the only material that J
remained was contaminated hoods and glove boxes which was possessed g?h.
under License No. 34-13517-01:(see Attachment B).
Subsequent telephone e
conversations were made between NRC Region-Ill' staff and Mr. Charles tg Wells (now deceased) in.the early 1980's prior to the termination of
- Q License No. 34-13517-01, in which Mr., Wells indicated that these h '.
contaminated items were buried in' two burials in accordance with 10 CFR 20.304 sometime in early 1980. 'These burials consisted of less 7
than 20.0 microcuries of americium-241 as contaminated equipment, metal, h
and concrete (see Attachment C). During this inspection, Mrs. Nancy b,
Wells (wife of the deceased), informed the inspector that she was not f,
aware of any burials of radioactive material on the property. Assuming g%
that the contaminated items were actually buried, the NRC staff perfcrmed a hypothetical dose assessment and pathway analysis using the i
computer modeling program RESRAD version 4.34). The conclusion to this jf hypothetical assessment was. based (upon,the following assumptions:
