The Gamma Industries Model A-13-A source assembly is: designed for Luserin the Gamma Industries Model 2-15SA exposure. device. The active material is metallic-Iridium-192, singly encapsulated in

type 304 or-316 stainless steel, using a heliarc_ welding technique. This source assembly-is a departure from the usual radiography source in that'it contains two (2) sources on a single pigtail, separated by a maximum distance of-four (4) inches (10.16 cm). The source capsules' closest to the connector in;the assembly _usesla~ side-weld closure, and the source capsule furthest from the connector in the assembly.uses an end-cap closure. The pigtail connector,. source capsules and locking ball are crimped to 1/8 inch (3.18 mm) stainless steel cable.

The source capsules ~are_not large.enough to allow' direct labeling. However, the exposure' device which contains the source capsules-is labeled; and for additional labeling information, please see the certificate of; registration andisafety analysis.

CONDITIONS OF NORMAL USE:
The Gamma Industries Model A-13-A source assembly is intended for
-use only_in the Gamma Industries Model 2-15SA Industrial Radiography exposure devices.
Gamma emission is checked against a standard using an ion chamber. A decay chart and source tag are supplied with each source, providing information concerning the activity and leak test results.

This' source assembly consists of a singly encapsulated universal
-end-weld. capsule and a singly encapsulated side-weld capsule on a l single pigtail. :Both capsules have.an ANSI classification of 1 77E43515, as described in ANSI' publication N542-1977. Based on prototype; test data, these, designs are expected to. retain containmentcintegrity under accident conditions likely to occur.

The-dose rate from unshielded source of iridium-192 is I approximately'5.9 R/hr per curie at 1 ft (30.48 cm).
? QUALITY' ASSURANCE AND' CONTROL:
The manufacturer. maintains pigtail dimensional tolerances at
-11/16 inch (il.59 rmn) . Each source assembly is subjected to, and
.must-successfully pass with no visible distortion, a 200 pound (90.72.kg) pull test prior to insertion of radioactive material.

.the results are doubtful 'then a hot water bubble test is performed. No source'is s shipped,if; removable contamination

. These sources shall.be leak tested.at intervals not to exceed six (6) months using techniques approved by the licensing authority and capable of detecting 0.005 microcurie (185 Bq) of removable contamination.

SUMMARY
The Model'A-13-A source is nollonger manufactured or distributed by Gamma Industries, .Inc. and Gamma Industries, Inc. is no longer in. business. Therefore,- the registration certificate is being converted to an inactive registration certificate.
The' source was originally approved for licensing purposes by the
. Louisiana Division of Radiation Control. NRC attempted to obtain copies of all information submitted by the registrant in support of:theLevaluation. However, NRC could not 1ocate all the '
information. Therefore, no safety evaluation of the source was
. performed'as part of the conversion of the certificate to
' inactive status, l
