DESCRIPTION 1 .

The Gamma Model 2-15SA is a right circular cylinder, 13 inches (33.02 cm) long and 6-~ inches (15.24 cm) in diameter. -Attached to

.one side of the cylinder and running the length of the device is a carrying handle. Attached to the side of the device opposite ,

the carrying handle is a base plate 5-1/2-inches (13.97 cm) wide. j At one end of the device,- 90 .from the carrying handle, is a Gamma Industries Type A lock box. At the other end and 180 opposite the lock box is an~ outlet' nipple.

]

The housing of the device is constructed from stainless steel and contains a right circular depleted uranium cylinder within which

,is cast a_zircaloy "S" tube. The casting and tube are a departure from the common radiography device.in-that they are l elongated.to an uncommon extent. This elongation is to provide

. shielding for a single source as'sembly which contains two (2) source' capsules with a maximum' source' spacing of 4 inches ,

(10.16.cm). The.83 pound depleted.. uranium shield is 3-5/8 inches

'(9.21 cm)iin diameter'and 11-7/8 inches (30.16:cm) in length.

The "S" shaped zircaloy tube is 0.500 inch ,(1.27 oc. m ) in diameter with a 5' inch (12.7 cm)/ straight:section and a 2 inch (5.08 cm) l radius 90 bend'on each end which protrudes.through the cylinde'r '

180 degrees apart. The depleted uranium casting is supported

-within the' stainless' steel. housing by 5 stainless steel shield j retainers. Four (4) stainless steel shield gussets are used to l prevent rotation of the shield within the device. Rotation of '

the shield' assembly.is further-precluded by engagement of the "S" tube into the lock assembly and the outlet nipple.  ;

.The combination of the locking : ball cn1 the pigtail assembly and ,

.the lock-insert in the lock box prevents the pigtail assembly l from coming through-the lock box' side of.the device. The locking

ball, in combination withfthe lock plunger when the lock is 1 depressed, prevent'the source assembly from coming through the outlet nipple. In addition, a safety plug is placed in the L outlet nipple.when not in use.

[ LABELING:

lThe device is labeled with the two (2) name plates and two (2)

" Caution,= Radioactive Material" -labels. See Attachment 2.

CONDITIONS OF NORMAL USE:

,The Gamma.Model 2-15SA is designed for use as a remotely controlled industrial radiography exposure device.

EXTERNAL RADIATION LEVELS: 1 A radiation profile was made of the device loaded with a pigtail assembly containing two-(2) 15-curie.(555 GBq) Iridium-192 sources separated by 4 inches (10.16 cm). -The readings were made using a-G-M survey meter _which was' calibrated against cesium-137.

The maximum reading on the surface of the device was found to be 50 mr/hr. The maximum reading at 6 inches (15.24 cm) from the surface of_the device was found to be 13'mr/hr, and no reading-

- above 1 mr/hr was found at one meter -(3.28 f t) .

QUALITY ASSURANCE AND CONTROL: l The' quali ty control procedure-for manufacturing the device is outlined in " Gamma Industries Quality Assurance Program for

. Radioactive Material Packages," U.S. NRC Approval No. 0010,

, _ : expiration date 11/30/84.

LIMITATIONS AND/OR OTHER CONSIDERATIONS OF USE:

. This device shall be possessed and used by persons specifically licensed by the U.S. NRC or an Agreement State.

. The license'should: include provisions for'83 pounds

.(37.7 kg) of. depleted. uranium.

  • The manufacturer furnishes step-by-step operating i instructions and step-by-step' instructions for periodic l maintenance of the device. However, it should be noted that this-device will be supplied to customers who intend to use  ;

it.in=a fashion different from that described in the )

manufacturers operating procedures. Therefore, the  !

, licensing agency must pay special attention to the I

licensee's operating and emergency procedures for use of I

this device.

l The manufacturer 1is aware of.at'least-one customer who intends to secureLthe drive cable.and pigtail assembly ')

within the source 7tube by,means of'a. modified Jacobs chuck '

at the disconnect. This is to allow the source tube to be removed from-the outlet nipple and-the. entire assembly, consistingLof-source tube, drive' cable and pigtail, to be pushed further into tubing. Gamma Industries supplies the modified Jacobs chuck and source' tube.

The manufacturerzstates'that the device meets Department of Transportation's Specification 7A, Type A, as a container.

  • Handling, storage, use, transfer, and disposal: To be

-determined by the licensing ~ authority.

. This registration sheet and the information contained within the references shall not be' changed without the written consent of the NRC.

- SAFETY ANALYSIS

SUMMARY

The Model 2-15SA device is no longer manufactured or distributed by Gamma Industries, Inc. and Gamma Industries, Inc. is no longer inibusiness. Therefore, the registration certificate is being converted to an inactive registration certificate.

' SAFETY ANALYSIS

SUMMARY

(Cont'd):

The device was originally approved for licensing purposes by the

? Louisiana Division of Radiation Control. NRC attempted to'obtain copies of all information submitted by the registrant in support

- of-the evaluation. However, NRC could not locate all the information. Therefore, no safety evaluation of the device was

- performed as'part of the conversion of the certificate.to

- inactive status.
