SUBJECT:

TRANSPORT OF A POTENTIALLY CONTAMINATED, INJURED INDIVIDUAL OFFSITE On May 13, 1987, at 6:11 pm (EDT), the licensee reported an Unusual Event, to the Hudquarters Operations Officer by telephone, in regard to a potentially contaminated injured individual who had to be transported offsite for medical attention due to a neck injury. On May 13,-1987, at 5:53 pm (EDT), a female air lock operator opened the air lock door at 93' level to transfer some material out of containment, and while doing so, the air lock door, by gravity, swung on to her neck. She opened the air lock door by herself and i she reported feeling dizzy, which the licensee felt could be due to possible head injuries.

The licensee performed a frontal radiation survey of her and transferred her, via ambulance, to St. John's Hospital in Longview, Washington, pursuant to their Emergency Plan Procedures.

i Subsequent radiation surveys indicated no contamination of the individual, ambulance or the

hsspital. X-rays at the hospital indicated no serious injury to the individual.

i Unusual Event terminated at 6:37 pm (EDT).
