4 NOTICE OF VIOLATION

. Commonwealth Edison Company Docket No. 50-456 Docket No. 50-457 As a result of the inspection conducted on October 7 through November 24, 1986, and in accordance with the " General Policy and Procedures for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1985), the following violation was identified

10 CFR 50, Appendix B, Criterion V as implemented by Commonwealth Edison Company (CECO) Quality Assurance Manual, Nuclear Generating Station, Section 5, requires that activities affee. ting quality be accomplished in accordance with documented instructions, procedures, and drawings of a type appropriate to the circumstances.

Contrary to the above, a deficiency in the submittal of Approval for Design and Installation (ADI) records into the L. K. Comstock, Inc., (LKC) Quality Records Vault was identified by LKC Quality Control. This deficiency was not recorded as a nonconforming item as required by LKC Procedure No. 4.11.1, Revision G, Paragraph 3.2 which states in part that "A Nonconformance Report, Form No.14, shall be originatea by Engineering or Quality Control Personnel on detection of deviations that conflict with specifications and/or drawings."

This is a Severity Level V violation (Supplement II).

The inspectien showed that action had been taken to correct the identified violation and to prevent recurrence. Consequently, no reply to the violation is required and we have no further questions regarding this matter.
