NOTICE OF VIOLATION Commonwealth Edison Company Docket Nos. 50-456;50-457 Braidwood Station I.icense Nos. NPF-72; NPF-77 i
Braceville, Illinois J
l As a result of an inspection conducted from October 19 through November 29,1996, two violations of NRC requirements were identified in accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," NUREG-1600, the violations are listed below:
1.
10 CFR Part 50, Appendix B, Criterion V, requires in part that activities affecting quality be prescribed by documented procedures of a type appropriate to the circumstances.
Contrary to the above, the construction of scaffolding on November 7,1996, in the vicinity of the essential service water system forebay was not prescribed by documented procedures appropriate to the circumstances. Specifically, BwAP 100-21, " Foreign Material Exclusion," Revision 0.1, prescribe appropriate controls and workers dropped scaffolding into a slot in the traveling screen housing that fell into the suction bay of the essential service water system.
This is a Severity Level IV Violation (Supplement I) (50-456/96019-02(DRP)).

Technical Specification 6.8.1.a requires that procedures be established, implemented, and maintained for activities covered in Appendix A of Regulatory Guide 1.33. Regulatory Guide 1.33, Appendix A, requires the use of procedures for the performance of maintenance that could affect the performance of safety-related equipment.
Procedure BwMP 3300-038, " Removal and Installation of the Primary Manway Cover on the Steam Generators (SG)," directed installation of 2 guide pins during manway installation and required lubrication of the female threads after removal of the guide pins and prior to installation of the last two bolts.
Contrary to the above, when the Unit 1 "C" SG cold leg manway was installed, the female threads were not lubricated after removal of the guide pins and prior to the installation of the last two bolts.
This is a Severity Level IV violation (Supplement l} (50-456/96019-03(DRP)).
Pursuant to the provisions of 10 CFR 2.201, Commonwealth Edison is hereby required to submit a written statement or explanation to the U.S. Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington D.C. 20555 with a copy to the Regional Administrator, U.S. Nuclear Regulatory Commission, Region Ill,801 Warrenville Road, Lisle, Illinois 60532, and a copy to the NRC Resident inspector at the Braidwood Station within 30 days of the date of the letter transmitting this Notice of Violation (Notice). This reply should be clearly marked as a " Reply to a Notice of Violation" and should include for each violation: (1) the reason for the violation, or, if contested, the basis for disputing the violation, (2) the corrective steps that have been taken and Notice of Violation 9703030113 970218 PDR ADOCK 05000456 G
Notice of Violation the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when fuli compliance will be achieved.

be issued as to why the license should not be modified, suspended, or revoked, or why such other action as may be proper should not be taken. Where good cause is shown, consideration will be given to extending the response time.
