1.0 DISC 1_SSION u .u-3 4 l The oijective of this procedure is to mitigate the censequences of a cavity [

seal failure during a period when the fuel gate is open and/or irradiated fuel is out of the core.  ;

t 2.0 SYMPTOMS

r 2.1 SPENT TUEL PIT LOW LEVEL Alarm.

2.2 REACTOR CONTAINNENT SUMP HI-LO LEVEL alarm and rapid increase on sump level recorder.

2.3 INCORE INSTRUMENT SUMP HI LEVEL alarm.  ;

2.4 Rapid increase on containment vide range level recorder.

l 2.5 Decreasing cavity level as indicated by digital display.

2.6 Possible RMS alarms from containment and/or spent fuel building.

t 2.7 Increase in stack release rate.  !

! 2.8 Decreasing cavity level as observed on staff gauge in cavity.

! 3.0 AUTOMATIC ACTION  !

l None . j I 4.0 IMMEDIATE OPERATOR ACTION 4.1 Sound containment evacuation alars.

1 4.2 Instruct refueling operators to immediately close the sluice gate in the Spent Tuel Building and to move any irradiated fuel assemblies to a shielded location (Attachment 'A' contains the immediate actions for .

the refueling operators).  !

E0P 3.1-48 Rovo 3 E0P1 5.0 SUBSEQUENT OPERATOR ACTION OCT 041984 5.1 Initiate blended make-up to cavity (small breaks).

5.2 Verify RHR system is in operation.

5.3 Igt not open RH-MOV-874 as this will cause pump down of the cavity.

5.4 Djl not open RHR sump suction valves.

5.5 Open the face dampers and start the CAR fans if available.

5.6 Close containment hatch inner and outer door and verify that penetrations providing direct access to the outside atmosphere are closed.

5.7 Stop containment purge and close the purge valves..

5.8 Monitor reactor vessel water level and verify that level decrease stops approximately at the vessel flange (0" on digital indicator).

5.9 Verify that refueling operators have isolated the spent fuel pool and all irradiated assemblies have been put into a shielded location.

5.10 Stop the spent fuel pit cooling pumps if the level in the pool decreases to a point near or below the pump suction to prevent pump damage.

5.11 Verify that incident is terminated es indicated by no further level increase on containment vide range ?.evel recorder.

5.12 Verify RCS is stable as indicated by RHR system in operation maintaining RCS temperature and no decreasing level in vessel as indicated by digital level indicator.

5.13 Notify H. P. Control Point at the containment hatch to limit access to the containment.

5.14 De-energize all non-essential electrical equipment in containment lower level.

3.15 Periodic make-up to the refueling canal using the purification pump and VCT or RWST as a source of water may be required to keep the canal full.

5.16 Restore the level in the pool if necessary using the purification purp and VCT or RWST aa a source of water and restart a SFP cooling pump.

CAUTION: Overboration of pool could result in bor(n precipitation.

Use low concentrations of blended cake-up or straight prdmary water of the pool is isolated from the cavity.

5.17 Proceed to recovery phase.

(Refueling Operators) i

1. Hove transfer car to containment side of fuel tube, shut sluice gate in l

spent fuel pool and attempt to shut gate valve unless radiation levels do  :

not permit.

1 NOTE: If service air is not available to close sluice gate, remove support pins using prying device provided.

2. Put away irradiated fuel assemblies as follows: '

(1) If assembly is ijl the core envelopt put back in core, unlatch the  !

assembly and move the manipulation crane away from the core, j (2) If not in the core envelope put in pre-determined spot in transfer

! canal all the wsy down to the floor beside the tracks. Unlatch the t assembly, return trolley to transfer canal zone, then move west at least two feet.

l (3) If in the fuel transfer car return it to containment but leave laying  !

down. ,

! (4) If irt the RCC change fixture leave it there. t i (5) If latched in the spent fuel pool, insert in fuel rack.  !

3. If an RCCA is withdrasa align the #3 RCC basket and insert the RCCA. f i

j 4 If fuel sipping is in progress and an assembly is in transit to or from the l sipping container, either put the assembly in the storage rack or in the i sipping container.
