Connecticut Yankee Emergency Operating Procedure No. E0P 3.1-21 Operations PRESSURIZER SPRAY VALVE MALFUNCTION 1.0 DISCUSSION This procedure outlines the action to be taken in the event of a malfunction of the pressurizer spray valves PR-A0V-573 and PR-A0V-574, in either the closed or the open position. Spray valve PR-A0V-573 is supplied from the cold leg of reactor coolant loop ~#4 while spray valve PR-A0V-574 is supplied from the cold leg of loop f 3. 10th spray valves are air-operated and their position is automatically controlled in proportion to pressurizer pressure between 2035 psig and 2085 psig. Manual control for each valve is also provided by switches on the main control board (MCB) which allows manual opening of the valves from the main control room. The air supply for the spray valves is provided by three air compressors and receivers located within the containment annulus.
The sequence of events that would occur if the spray valves fail in the closed position and allow pressurizer pressure to raise or if they fail in the open position causing pressurizer pressure to drop are tabulated below.
2350 psig - pressurizer relief valve opens 2300 psig - pressurizer hi pressure reactor trip 2085 psig - spray valve full open 2050 psig - pressurizer hi pressure alarm 2035 psig - spray valves begin to open 2000 psig - nominal pressure 1950 psig - pressurizer lo pressure alann all backup heaters on less than 1700 psig - reactor trip 2.0 SPRAY VALVE MALFUNCTION IN THE CLOSED POSITION 2.1 Synptoms 2.1.1 Increasing pressurizer pressure indication on the pressurizer pressure recorder above the nominal 2000 psig with no subsequent decrease above 2035 psig.
2.1.2 High pressurizer pressure alarm at 2050 psig.

2.1.4 One or both spray valves indicating lights on the main i
control board shows that the valve (s) is closed.
2.1.5 Low containment inst, air pressure alam.
2.1.6 CAR fan dampers shift to accident mode.
2.2 Automatic Actions 2.2.1 Pressurizer pressure control output signal calling for spray valves open (greater than 50% output signal).
2.2.2 Power operated relief valves open at 2350 psig.
All pressurizer Automatic reactor trip occurs at 2300 psig). backup heaters off a 2.2.3 2.3 Imediate Operator Action 2.3.1 Take manual control of pressurizer heaters and spray to maintain system pressure.
2.3.2 Adjust Tavg as necessary to control system pressure.
2.3.3 If system pressure cannot be maintained, perfom imediate actions contained in E-0 Reactor Trip or Safety Injection.
2.4 Subsequent Operator Action 2.4.1 Check containment inst, control air pressure; if low, check corrpressor power supplied (MCC7 and 8).
2.4.2 Call Duty Officer, and file a PIR as required by ACP 1.E-6.1.
3.0 SPRAY VALVE MALFUNCTION IN THE OPEN POSITION 3.1 Symptoms 3.1.1 Decreasing pressurizer pressure below the nominal 2000 psig as indicated on the pressurizer pressure recorder with spray valve switch indicating lights showing one or both spray valves open.
3.1.2 Low pressurizer pressure alam at 1950 psig.
3.1.3 All back-up heaters on at 1950 psig.
3.1.4 Computer alam message - Pressurizer pressure low on channels 1, 2, 3, and 4 3.1.5 PIC-401-1 Control signal failure alam.

3.2.1 Pressurizer pressure control output signal indicating control heaters on at full output (output signal less than 10%).
3.2.2 Automatic reactor trip from variable low pressure trip calculator (equal to or greater than 1700 psig).
3.3 Immediate Operator Actions 3.3.1 Take manual control of pressurizer heaters and spray to maintain system pressure.
I 3.3.2 Adjust plant load as necessary to control system pressure.
Idle the loop with the failed open spray valve as per l
NOP 2.4-3.
3.3.3 If system pressure cannot be maintained, perform innediate actions contained in E-0 Reactor Trip or Safety injection.
3.4 Subsequent Operator Action 3.4.1 An entry into containment must be made to isolate the inoperable spray valve.
3.4.2 If RCS pressure is less than 1700 psig, check that core i
cooling has actuated and is operating. Go to E-0 Reactor Trip or Safety Injection.
3.4.3 Call Outy Officer and file a PIR as required by ACP 1.2-16.1.
