Grand Gulf Nuclear Station Units 1 and 2 Docket Nos. 50-416/417 File 0260/15525/15526 PRD-81/45, Interim Report No. 1, Transamerica Delaval Standby Diesel Generator AECM-81/482 On November 10, 1981, Mississippi Power & Light Company notified Mr. P.

A. Taylor, of your of fice, of a Potentially Reportable Deficiency (PRD) at the Grand Gulf Nuclear Station (GGNS) construction site. The deficiency concerns spherical washer subassemblies on the Transamerica Delaval Division II Standby Diesel Generator.

During our investigation of this matter, additional deficiencies were identified. These deficiencies are still under investigation.

The determination has been made that this deficiency, pertaining to the spherical washer subassembly, had it gone uncorrected, could have had an adverse affeet on the safe operations of the plant. Therefore, this deficiency is reportable under the provisions of 10CFR50.55(e). The determination of the applicability of 10CFR21 is being investigated.

Our Interim Report is attached. We expect to submit a Final Report by March 15, 1982.

Attachment to AECM-81/482 Page 1 of 1 INTERIM REPORT NO. 1 FOR PRD-81/45 I. Description Of The Deficiency During startup testing of the Division II Standby Diesel Generator a piston crown partially separated from the piston skirt due to the failure of one of the four (4) attachment stud bolts. The stud bolt failure could have likely been caused due to a deficiency of the spherical washer subassembly.

Additional deficiencies were identified during our investigation of the spherical washer subassembly deficiency. They are considered to be generic and deal with a missing wrist pin keeper, grooves in cylinder liners and excessive bearing wear and deterioration. The cause and extent of these deficiencies are still under investigation.

The spherical washer subassembly deficiency is applicable to Unit 1 and Unit

2. The determination has been made that if this deficiency had gone uncor-rected, the failure of the stud bolts could render the engine inoperable which could have had an adverse ef fect on the safe operations of the plant.

Therefore, this deficiency is reportable under the provisions of 10CFR50.55(e).

II. Approach To Resolution Of The Problem The Transamerica Delaval investigation to determine the cause of the deficiency is incomplete; but their current position, is that the stud bolt failure was likely due to either improper heat treatment or incorrect machining of the spherical washer subassembly.

The pistons from the Unit I diesels, including spares, have been returned to Delaval for modifications aad the potentially defective spherical washers are being replaced with Belleville washers. Also, following mag particle examina-tion, which provides additional assurance that they are free of flaws, replace-ment stud bolts are being installed.

III. Status Of Proposed Resolution The Transamerica Delaval failure analysis is incomplete at this time. It is expected that the failure analysis will be complete by mid-December.

, IV. Reason Why A Final Report Will Be Delayed i

Our Final Report will be delayed due to the fact that the specific cause of the spherical washer subassembly deficiency and the cause and extent of the addi-tional possibly generic deficiencies identified above, are still under investi-gation.

l V. Date When A Final Report Will Be Submitted Mississippi Power & Light Company expects to submit its Final Report on or before March 15, 1982.
