Cantrell, of your office, of a Potentially Reportable Deficiency (PRD) at the Grand Gulf Nuclear Station (GCNS) construction site. The deficiency concerns the failure of a test connection pipe off the HPCS injection line on the containment side of the RPCS injection valve (QIE22-F004)

We have not completed our investigation to determine reportability. We expect to submit a final report by October 29, 1981.

1. Description of Deficiency While running HPCS vessel injection testing at 40% at rated flow for the collection of pipe vtbration data, the test connection on the containment side of valve F004 was found broken off. The small pipe break occurred between valve F021 and the branch cennection to the injection piping.

II. Approach to Resolution of the Preblem A. An investigation is to be conducted to determine:

1. The cause of the deficiency.
2. The extent of the deficiency.

B. Based on the results of the above investigation, an evaluation will be made to determine:

1. Reportability under 10CFR50.55(e) and 10CFR21.
2. Corrective actions to preclude recurrence.
3. P.emedial actions to correct this deficiency and others, if any, identified during the investigation.

III. Status of Proposed Resolution The investigation is in progress by our Architect / Engineer. To date, neither the cause nor the extent of the deficiency have been determined.

It does appear, however, that the deficiency is limited to the non-NSSS scope of supply. .

IV. Reason Why a Final Report Will Be Delayed The investigation into the cause and extent of the deficiancy has not been completed.

V. Date Whea a Final Report Will Be Submitted We expact to submit a final report by October 29, 1981.
