Response-to Inspection Report 50-271/89-17 l Routine safety inspections were conducted at Vermont Yankee from September S - October 16, 1989. The subject Inspection Report [ Reference b)] requested that we respond to the_NRC-in writing with our_ assessment and identification of J additional-corrective action implemented or contemplated regarding the issue of degraded or. defeated locked doors. Our response to this request is as follows. 1 We have re-reviewed each of the incidents identified in Reference'b)'to attempt to identify if additional corrective actions should have been taken at the time of_the event. The results of this review indicate that, based on the specifics of each individual event, the corrective actions were timely and appropriate. However, in light of the repeat instances of similar events, we '

a feel that additional steps are warranted. Primarily based on the results of our evaluation of the Reactor Cleanup door incident, the-following additional measures have been recommended and/or completed:

o- By January 31, 1990, additional meetings between the Plant Manager, depart-ment supervisors and personnel will take place to review the most recent door incident. The focus of the meetings will be to insure that super-visors are aware of their responsibility for insuring administrative controls for personnel protection / safety are complied with.and that all personnel are aware of their accountability with respect to this issue.

o Appropriate plant procedures will be revised to clarify management's expec-tations with regard to maintaining control of locked doors. At a minimum, the criteria for which doors require locking will be established. This effort will be completed by' Jar.uary 31, 1990.

t o A surveillance program for administrative 1y controlled doors will be insti-

-tuted by February 28, 1990. The intent of this program will be to ensure 7

'that door operation and closure are satisfactory. It is anticipated that this surveillance will be included as part of the existing safety inspec-tion program.-

We agree with the NRC that it is essential to comply with administrative controls which enhance radiation protection. We feel that completion of the actions identified above will further strengthen our program and minimize the potential for repeat events.
