Unscheduled SIS Cable Pull AECM-81/162 On February 11, 1981, Mississippi Power & Light Company notified Mr. J.

Rausch, of your office, of a Potentially Reportable Deficiency (PRD) at the Grand Gulf Nuclear Station (GGNS) construction site. The deficiency concerns the installation of unscheduled Single Insulated Stranded (SIS) wire without a specific Work Plan and Inspection Report (WP&IR) or documentatio1.

This deficiency does not meet the criteria of reportability under 10CFR50.55(e) or 10CFR Part 21. Attached is our final report on this matter.

Description of the Deficiency Terminal boxes were installed in accordance with our constructor's l specification on valves where the vendor had supplied services with no l way to terminate one cable to these devices. The vendor had no requirement to supply such a box. Our Architect / Engineer issued standard details to allow for the installation of terminal boxes. Work Plan and Inspection Reports (WP& irs) were issued for the unscheduled Single Insulated Stranded (SIS) wire. The SIS wire is considered an extension of the scheduled cable and was inspected with the scheduled terminations but no objective evidence could be found to document the inspection. During a re-write of Work Plan / Procedure (WP/P) E-22,

, approved May 5,1980, a statement was added to require the documentation of the " unscheduled" SIS wire on the termination of WP&IR for the scheduled termination. A re-write of WP/P-E-33, approved December 29, 1980, added a line item to the terminal box WP&IR for inspection and documentation of " unscheduled" SIS wire. A vault audit was pertormed to determine the extent of the problem and identify all valves installed prior to May 5, 1980. The constructor determined that a maxicum of 225 valves could have undocumented SIS wire installed.

II. Analysis of Safety Implication The cited condition has been determined to have no adverse af fects to l

the safety of plant operations. Therefore, the deficiency is not reportable under 10CFR50.55(e) and 10CFR21.

! Investigative results indicate that the undocumented wiring installation and connections were performed in an acceptable manner. There were no hardware deficiencies. Therefore, had the cited condition remained uncorrectel, this event would have no adverse affects on the safety of plant operations.

III. Corrective Actions Taken, I

Inspection of the 225 valves, which could have been affected by this deficiency, produced only 11 valves which had undocumented wire installed, and the actual wiring was acceptable in all instances.

The following actions have been taken to prevent recurrence: WP/P-E-22 was revised December 29, 1980 to add a line item on the terminal box WP&IR for the inspection and documentati sn of "unschaduled" SIS wire; and documented training sessions were held to inform the involved parties about the revised procedures and application of WP/P-E-22 and WP/P-E-33.
