SU3JECr: Grand Gulf Nuclear Station Units 1 and 2 Docket Nos. 50-416/417 File 0260/15525/15526 PRD-81/21, Final Report, Unauthorized Replacement of Motor Connection Box AECM-61/146 On March 20, 1981, Mississippi Power & Light Conpany notified Mr. P. A.

Taylor, of your office, of a Potentially Reportable Deficiency (PRD) at the Grand Gulf Nuclear Station (GGNS) construction site. The deficiency concerns replacement of a socor connection box on a Standby Gas Treatsent Systes filter, by our Constructor, without proper documentation.

Our investigations have resulted in our determination that this deficiency is reportable under 10CFR50.55(e) and is not reportable under 10CFR21. Attached is our final report.

o Attachment to AECM-81/146 Page 1 of 1 FINAL REPORT FOR PRD-81/21 I. Description of the Deficiency This deficiency concerna a motor connection box in the Standby Gas Treatment System (T48). The original box supplied by the vendor was misplaced and our Constructors substituted a different motor connection box. This item was installed on filter aesembly Q1T48D001A-A.

Investigations determined there was no objective evidence of. approval by our Architect / Engineer for installation of the Bechtel supplied connection box.

This deficiency concerns only one (1) motor connection box in Unit 1.

It is not applicable to Unit 2.

II. Analysis of Safety Implications The deficient connection box was larger than the vendor supplied box.

The larger box could af fect the seismic qualification of the related equipment. Lack of seismic qualification could compromise integrity / function of the Standby Gas Treatment System during a seismic event.

III. Corrective Actions Taken Corrective actions include replacement of the Bechtel connection box with a vendor suppl.ied box as per specifications. Also, our Constructor has issued a " Job Bulletin" to emphasize proper procedures for substitution of components. This deficiency is considered to be an isolated case of failure to properly follow " substitution of components" procedures.
