Dear Mr. O'Reilly:

SUBJECT:

Grand Gulf Nuclear Station Units 1 and 2 Docket Nos. 50-416/417 PRD-80/49, Guyon Coupling Linear Indications, Interim Status Report No. 2 AECM-80/277 On August 11, 1980, Mississippi Powe. & Light Company notified Mr. M. Hunt of your office of a Potent 1'lif Reportable Deficiency (PRD) at the Grand Gulf Nuclear Station (GCNS). The deficiency concerns indications on couplings supplied by Guyon Alloys, Inc.

for use in nuclear Class I & II applications.

We have noc yet completed cur review of this condition. An interim status report is attached. We estimate that our final report will be aubmitted by March 25, 1981.

Yours trulv, f f f/ / J. P. McGaughy, Jr.

' Attachment to AECM-80/277 Page 1 of 1 INTERIM STATUS REPORT FOR PRD-80/49 I. Description of Deficiency Linear indications were found in four nuclear Class II and two nuclear Class I, three quarter inch (3/4") stainless steel 304L couplings. The linear indications were in the face and along the body of the fittings. These couplings were supplied by Guyon Alloys, Inc. The heat code and heat number for these couplings were QE and A17130, respectively.

II. Resolution of the Deficiency The six couplings described in the " Description of the Deficiency" were determined to be in the sensing lines on the Reactor Pres-sure Vessel instrumentation piping. An investigation determined that loss of the sensing lines in which these six couplings were located would result in automatic shutdown of the reactor.

However, MP&L has determined that many other similar Guyon couplings having the same heat number have been shipped to the GGNS site. The Constructor is presently in the process of locating these couplings and performing a non-destructive examination of them.

III. Status Although couplings have been identified as used in safety-related systems, it has been determined that their failure would not adversely affect the safety of operations of the plant. However, until the location of all the received couplings has been determined, the potential exists for their use in a safety system where their failure could adversely affect the safety of plant operations.

IV. Reasons for Delay in Final Report This deficiency is still under investigation.

V. Projected Completion Date A final report will be provided by March 25, 1981.
