Dear Mr. Keppler:

This letter constitutes the final report required by loCFTt50.55(e) on the deficiencies concerning Cutler-Ha==er motor control centers. These ite=s were first reported to the NRC by Cutler-He==er as a loCFR21 on May 11, 4 1980. It was later reported as a significant deficiency under loc 7R50.55(e) in a telephone conversation between W. J. Kacer and T. A. Boss of The Cleve- i land Electric Illuminating Company and Mr. J. Konklin of the NRC, Region III, I office of Inspection and Enforce =ent on May 28, 1980 An interi= report was l later submitted to you en June 25, 1980. l l

Description of Deficiency Cutler-Ha==er was contracted by The Cleveland Electric Illu=inating Company i (CEI) to manufacture Class IE notor control centers for Perry Nuclear Power Plant. During a final inspection by Cutler-Ha==er of a portion of these units, it was discovered that the wire installed in so=e of the starter units was not qualified to IEEE 383 and that improper saddle clamps were used on the terminal blocks. Further inspection of e l 1,943 starters revealed that 42 contained i= proper wire and 25 contained ter=inal blocks with i= proper saddle clamps. It was later discovered at the job site that four splice plate carriage bolts broke during installation on the bus.

Discussion of this proble= with Cutler-Ha==er representatives revealed that during a configuration study it was discovered that the wire being used did not meet specification requirements. In addition, it was discovered that inadequate production floor controls enabled i= proper ter=inal blocks to be installed in the units. Exa=ination of the carriage bolts by CEI's labora-tory showed that the bolts failed due to a =anufacturing defect which probably affected an entire production batch.

Due to the number of motor control centers received with i= proper wiring, i= pro- ,

per terminal blocks and defective carria6e bolts, there is some concern that the units win be able to perform adequately during or after a seismic event or loss of coolant accident. Since the motor control centers are necessary for the safe operation and shut down of the nuclear power plant, extensive repair v41' be nec-essary to bring the centers into compliance with the specification.

Corrective Action In order to correct the deficiencies in the cotor control centers, the following steps will be taken:

1. Those starters with i= proper wire will be returned to Cutler-Hammer to be rewired with prequalified type SIS control wire.

This win bring the starters' wiring into ec=pliance with the specification.

2. All terminal blocks with i= proper saddle cla=ps win be re-placed either at Cutler-Ha=mer or at the site, depending on whether or not the particular starter has been returned to Cutler-Ha==er for rewirir4 3 An splice plate carriage bolts will be replaced. In order to ensu;e the bolts win be adequate, Cutler-H-r win test an eight percent sampling of all replacement bolts in accor-dance with MIL-STD-105-D. If any bolts fail the test, Cutler- i Ha==er will reject the batch. To verify replacement at the i site, all replacement bolts will be die sta= ped upon receipt s.nd bolt replacement will be documented by the installing con-  ;
