Attached is the final report pursuant to 10CFR Parts 21 and 50.55(e) concerning the high voltage power circuitry supplied by Kaman Instrument Co. for use in the Perry Nuclear Power Plant (PNPP) Units 1 and 2 Post Accident Radiation Monitoring Systems (D19).
Mr. W. Forney of your office was verbally notified March 26, 1986, that this was being evaluated per Deviation Analysis Report 262.
Our initial 10CFR21 report for PNPP Unit 1 was filed on April 3, 1986, and our initial 10CFR50.55(e) report for PNPP Unit 2 was filed on April 25; 1986. This report furnishes all information required by 10CFR21.21(b)(3) and meets the intent of 10CFR50.55(e)(3).

-The power supply voltage was found to be oscillating between 240 and 1030 volts at 100 HZ. during Unit 1 troubleshooting activities.
Investigation was conducted by disconnecting the mineral insulated (MI) cable and the detector from the high voltage power supply.
With the power supply set to operate at 800 volts, the power supply output voltage was found to spike to 1300 volts when the pcwer was turned on to the microprocessor. This voltage spike exceeds the manufacturers hypot test voltage of 1000 volts for the MI cable and causes a momentary insulation failure in the cable, which results in the oscillating voltage. Continued voltage oscillation eventus11y causes complete MI cable failure. This problem also applies to the Unit 2 equipment.

-The MI cable failure results in a voltage signal which could reduce the range of radiation detection. An erroneous indication from the detectors would not provide accurate quantitative information to plant personnel for determination of accident severity in the containment and drywell.
It should be noted that other means are available to ascertain post accident radiation in these areas.

The Number and location of all such components obtained for Perry Plant.
-The Kaman Post Accident Radiation Monitor Microprocessor has a total of two power supply circuit boards, each Plant Unit has two Microprocessors.
Therefore, a total of four high voltage power supply cards are installed in PNPP Unit 1 Control Complex - E1. 638' (1D19J100 and 1D19J200). The four cards supplied for PNPP Unit 2
equipment are stored in the Project Organization warehouse.

-Nonconformance Report PPDS-835. TAS-125 and Field Change Request 01722 were generated to track the nonconformance and to provide a means for resolution upon completian of the Project Organization engineering investigation. The corrective action relative to the closure of the Unit 1 Nonconformance Report, PPDS-835, has been completed. These actions included return of the affected circuit board cards to Kaman Instrumentation for circuitry modification which limits the voltage spike on system startup to less than 1000 VDC, and retest of the system to verify correction of the condition as reported. The corrective actions pecessary for resolution of Nonconformance Report TAS 125 and the closure of the Unit 2
deficiency will be completed consistent with the Unit 2 construction schedule.
