Enclosed is a copy of the written report concerning Kaman liigh Voltage Power Supply Circuitry cupplied to the Perry Nuclear Power Plant (PNPP).
Mr. W. Forney of your office was verbally notified on March 26, 1986 that this item is being evaluated per Deviation Analysis Report 262.
This report is being cubmitted pursuant to 10CFR21 for PNPP Unit 1.
Verbal notification was made to the NRC because a potential generic implication may exist for other nuclear pcwer licensee to whom this component has been cupplied. Evaluation of the information required by 10CFR21.21(b)(3) ic ctill ongoing which has led to the submittal of this written report outside the time frame identified in 10CFR21.

-Kanan High Voltage Power Supply (P/N 450447) in Microcomputers 1D19-J100 and 1D19-J200 for the Post Accident High Range Radiation Monitoring System at Perry Nuclear Power Plant Unit #1 (Control Complex E1. 638'-0).

The nature of the defect or failure to comply and the safety hazard which is created or could be created by cuch defect or failure tc comp 1.

-The power supply voltage was found to be onci11ating between 240 and 1030 volts at 100 HZ. during troubleshooting activities.
Investigation was conducted by disconnecting the mineral insulated (MI) cable and the detector from the high voltage power supply.
With the power supply set to operate at 800 volts, the power supply output voltage was found to spike to 1300 volta when the power was turned on to the microprocecoor. This voltage apike exceeds the manufacturers hypot test voltage of 1000 volta for the MI cable and causen a momentary insulation failure in the cable, which results in the oscillating voltage.
Continued voltage oscillation eventually causes complete MI cabic failure.

-The MI cable failure results in a voltage signal which could reduce the range of radiation detection. An erroneous indication from the detectors would not provide accurate quantitative information to plant personnel for determination of accident severity in the containment and drywell.
It should be noted that other means are available to accertain post accident radiation in these areas.

The date of receipt of information reasonably indicating that a condition reportable under 10CFR21 has been identified at PNPP.

The Number and location of all such componenta obtained for Perry Plant.
-The Kaman Post Accident Radiation Monitor Microprocessor has a total of two power supply circuit boards, each Plant Unit has two Microprocessors. Therefore, a total of four high voltage power supply cards are installed in PNPP Unit 1 Control Compicx - El 638' (1D19J100 and 1D19J200). The four cards supplied for PNPP Unit 2 equipment are stored in the Project Organization varchouce.

The corrective action which haa been, is being, or will be taken; the naec of the individual or organization responsible for the action; and advice concerning the length of time that has been or will be taken to complete the action.
-Nonconformance Report PPDS-835 TAS-125 and Field Change Request 01722 have been generated te track the nonconformance and to provide a means for resolution upon completion of the Project Organization engineering investigation.
Currently, Project Organization and the vendor are working together to determine the cauce of the nonconformance and the corrective action for these power supplies. Corrective action has already included replacement of the failed MI cables. All corrective actions and repair of the components will be completed prior to exceeding 5% power, consistent with the schedule for completion of preoperational testing. Thic completion milectone hac previously been documented in our February 27, 1986 letter (PY-CEI/0IE-178L).
