r e n r_= e~,aL L'v es As a result of investigation based on IE Circular 79/17, Eechtel has 12 identified eight (8) switchgear units containing defective S3-12 switches.

r ach switchgear is furnished with two (2) SB-12 switches. The switches Us contain an inter =ittent contact proble uhich will cause a lack of continuity through the contact of the switch used to energize the trip ,

- coil of the circuit breaker. The switchgear units are used in the 6.9 KV system used for controlling the recirculating pu=ps.

1. Descriotion cf Deficiener Defective S3-12 switches identified in IE Circular 79-17 vere found to be installed in switchgear supplied by tha vendor.

~he defect is an inter =ittent contact prcble: which will cause lack cf centinuity through the "n" contact'of the switch that is used to enargize the trip ceil of the circuit breaker.

II. Safetv inolications .

The 53-12 switches are used in the circuit breakcrs of the 6.9 l'V switchge t units which control the reactor recirculation pu ps. The safety ft_nction cf the breaker is to . trip on a signal frc: the Reacter Protection System. The pcstulated failure of the S;-12 switch would prevent the e:iergining of the trip coil thereby preventing the breaker frc: fulfilling its intended safety functien.

III. Cerrective Action Taken The vendor will furnish new S3-12 rwitches to correct the condi-tion and preclude recurrence, i
