DISCHARGE OF HYDRAULIC OIL FROM PUBLIC SERVICE ELECTRIC AND GAS (PSE&G) ARTIFICIAL ISLAND (CASE #91-11-08-1416-34) - 30 DAY CONFIRMATION REPORT Per the requirements of NJAC 7:1E-5.8, PSE&G has prepared the attached report confirming the discharge of 5 gallons of hydraulic oil from the outrigger of a PSE&G 80 ton crane to the ground. The discharge, which occurred at the above facility on November 11, 1991, occurred when the hydraulic oil line of the outrigger was pinched and ruptured. The incident took place entirely within PSE&G property and no injuries resulted.

Since there is no "Reportable Quantity" (RQ) for hydraulic oil under 40 CFR 302.4, no notification of the release was reported to any agency other than the New Jersey Department of Environmental Protection and Energy (NJDEPE) and the Nuclear Regulatory Commission (NRC), as required under 10 CFR 50.72, Significant Events.

PSE&G believes that it acted in accordance with the intent of NJAC 7:1E, Subchapter 5, which requires all major facilities to notify the NJDEPE of discharges of petroleum and other potentially hazardous substances in any concentration. As a result of this discharge, PSE&G has investigated the incident, with the information contained herein.

Hydraulic oil line from the outrigger of an 80 ton crane.

Approximately 5 gallons of hydraulic oil from a ruptured hydraulic line discharged to the ground. The failed oil line provided hydraulic oil to the outrigger of an 80 ton crane, which allowed the crane to function. The event occurred during routine activities, as the crane was preparing to lift a structure and some large equipment.

CASE #91-11-08-1416-34 This is the second hydraulic oil discharge from the same source to occur within 30 days. Design problems with the crane outrigger were noted in the discharge report for the previous Case (Case #91-10-18-0923-05). In that report it was noted that "The design location of the hydraulic line, located within the outrigger assembly, precluded any foreknowledge that the oil line was in danger of rupture."

After that incident, the crane was removed from service and both hydraulic oil hoses were replaced with the same length and type of hose. The crane was then returned to service at the same location of the- original discharge. There, the outrigger operation apparently pinched and ripped open the hoses, causing the discharge of an estimated 5 gallons of hydraulic oil. Again, the physical location of the hoses inside the outrigger precluded knowledge that the hoses were being pinched.

MEASURES TAKEN TO CONTAIN, CLEANUP AND REMOVE THE DISCHARGE:

Personnel normally assigned with the responsibility of containment, cleanup and removal of soil and gravel contaminated with petroleum and petroleum by-products are the same personnel tasked with the responsibility for heavy equipment operation. Therefore, many individuals were present at the discharge location when the event occurred.

The draining hydraulic line was quickly secured, stopping any further discharge. The affected soil and gravel (approximately 5 cubic feet) was placed into a 55 gallon drum for disposal.

Maintenance work performed on the crane outrigger was undertaken with the use of a large drip pan placed under the hydraulic oil line, to prevent further drips or leaks. All cleanup activities were completed by November 8, 1545 hours0.0179 days <br />0.429 hours <br />0.00255 weeks <br />5.878725e-4 months <br />.

CORRECTIVE MEASURES:

The cause of the discharge was determined to be the ruptured hydraulic hose. As noted, the location of the hose within the outrigger assembly precluded any foreknowledge that the hose would rupture. An examination of the outrigger assembly and the hydraulic oil hoses was conducted. It was determined that shortening the hoses would significantly decrease the possibility of another reoccurrence since the pinching occurred because of excessive hose length.

As was mentioned in the previous discharge report "Maintenance of the outrigger is an extensive process involving significant disassembly. Problems with the hose locations inside the assembly, particularly with the absence of extra room for the hoses during outrigger operation was noted."
