DISCHARGE OF WATER - UNDERGROUND PIPE LEAK PUBLIC SERVICE ELECTRIC AND GAS (PSE&G)

SALEM GENERATING STATION -

(CASE #91-11-21-0918-06) - 30 DAY CONFIRMATION REPORT As per the requirements of NJAC 7:1E-5.8, PSE&G has prepared the attached report confirming the discharge of water originally reported as a broken line on #2 Oil/Water Separator (skim tank) .

The discharge, which occurred at the above facility on November 21, 1991, occurred entirely within PSE&G property and no injuries resulted.

As 'the leak and discharge was not from a known polluted or radiologically contaminated system, no notification of the release was reported to any agency other than the New Jersey Department of Environmental Protection and Energy (NJDEPE) and the Nuclear Regulatory Commission (NRC), as required under 10CFRS0.72, Significant Events.

The leak was in fact determined to be the result of tidal water in the reinforced concrete underground discharge piping leading from a permitted treatment works which was out of service at the time and. not discharging. At the time of the incident, the river was at high tide accounting for the presence of the water in the discharge piping. Excavation activity disturbed the discharge piping, dislodging approximately 6 inches of pipe sealant/packing and allowing the river water leakage into the excavation.

NJ DE PE 2 NLR-E91307 PSE&G believes it acted in accordance with the intent of NJAC 7:1E, Subchapter 5, which requires all major facilities to notify the NJDEPE of discharges of petroleum and other potentially hazardous substances in any concentration. However, based on a review of system operations and the discharge event, the discharge of water was not from the facility but in fact to the facility from the river. Therefore, PSE&G was not obligated to notify the NJDEPE since the event was not a hazardous substance discharge.

As a result of this discharge, PSE&G has conducted a follow-up investigation of the incident, has repaired the piping leak and ta~en further action to initiate an engineering review of the system piping to determine the best means of lining the piping to minimize the potenti_al for future recurrence.

  1. 2 Oil/Water Separator discharge piping The leak was the result of a disturbance to the oil/water separator underground piping caused by excavation activity. The activity dislodged approximately 6 inches of pipe sealant/packing allowing the river water leakage into the excavation.

This treatment works was out of service at the time and not discharging.

CASE #91-11-21-0918-06 This discharge occurred after a disturbance to a section of underground piping. As a sump was dewatering the excavation at the time of the discovery, quantification of the amount of water from the pipe is .not practical. Approximately 5 cubic feet of soil from the immediate area of the piping leak which was removed after the discovery of the leak, was treated as a potentially oil contaminated soil and drummed for disposal.

Personnel discovering discharge were unable to determine probable cause for reportability. Licensing and Regulation personnel responded to remote location to make on-site determination of reportability. Immediate reporting was then made.

MEASURES TAKEN TO CONTAIN, CLEANUP AND REMOVE THE DISCHARGE:

The sump already in place to dewater the excavation continued to operate and discharge to a permitted treatment facility (#1 Oil/Water Separator) and subsequently to outfall DSN 489. No water left the immediate vicinity of the excavation. The discharge was terminated by the installation of a temporary packing.

CORRECTIVE MEASURES:

After the discharge was discovered, the Shift supervisor verified the inoperable status of #2 oil/water separator. The construction manager responsible for the site directed craft personnel to apply the temporary packing to the leak allowing subsequent repair by encasement of the joint in a concrete sleeve poured over the joint in the reinforced concrete pipe.

  • An adjacent potential joint leak on the piping was unearthed and a "hot patch" was applied to ensure that i1: would not leak after closure of the excavation.

To further prevent future recurrence of a similar problem, Licensing and Regulation sponsored a Nuclear Department Resource Allocation Program request to investigate engineering solutions to prevent future discharges of this nature on this system.

No significant cleanup activities were conducted associated with this discharge. on-site personnel with contaminated soil disposal responsibilities were consulted for this discharge cleanup. Approximately 5 cubic feet of soil, removed from the immediate area surrounding the pipe joint was handled as hazardous as a precautionary measure. These individual(s) are:
