At 1607 hours0.0186 days <br />0.446 hours <br />0.00266 weeks <br />6.114635e-4 months <br /> on 8/27/91, a continuous release from the Secondary Waste Sample Tank (SWST) was initiated. Technical Specifications 3.11.1.1 Table 4.11-1 Action 2.b requires a continuous composite sample proportional to the discharge flow rate from the SWST while conducting a continuous release. Site Chemistry personnel were notified at this time to start the composite sampler and adjust the flow rate corresponding to the release rate. The Chemistry Technician responsible for this action became distracted with other shift duties and did not initiate the required sample. The release was secured at 2345 hours0.0271 days <br />0.651 hours <br />0.00388 weeks <br />8.922725e-4 months <br /> and was discovered at this time that the sampler had never been started.

it The cause of this event was personnel error on the part of the Chemistry Technician due to not initiating a composite sample. Immediate corrective actions vere to verify that the effluent stream contained no radioactivity. This was confirmed by the fact that the SWST radiation monitor, which was functioning properly during the release, did not detect any abnormal activity levels.

Results of an iso'topic analysis that had been performed on this water prior to being transferred to the SWST vere also reviewed. These results also indicated no abnormal activity levels. Additional corrective actions will include a procedure revision and a review of this incident with applicable personnel.

At 1607 on 8/27/91, with the plant operating at 100% power, Radwaste Operations personnel were making preparations for a continuous release of the Secondary Waste Sample Tank (SWST). Plant Operating Procedure OP-120.01 requires notification of Chemistry personnel so the ISCO composite sampler can be started and the flow rate adjusted to the corresponding release rate. This notification was performed, but the chemistry technician responsible for this action became distracted with other shift duties and did not start the ISCO composite sampler.

At 2345 hours0.0271 days <br />0.651 hours <br />0.00388 weeks <br />8.922725e-4 months <br />, upon completion of the release, Chemistry personnel discovered this problem when the sample was to be secured.

CAUSE The cause of this event was personnel error on the part of the chemistry technician. Proper notification was made by Radwaste Operations but the technician became distracted with other shift duties and failed to initiate the composite sample.

SAFETY SIGNIFICANCE There were no safety consequences as a result of this event. This is based on the fact that the SWST radiation monitor was functioning properly during the release and did not detect any radioactivity in the effluent stream. In addition, an isotopic analysis was performed on the water just prior to transferring was present.

Immediate Corrective actions were to verify that no radioactivity was present in the SWST effluent stream. This was confirmed by the fact that the SWST radiation monitor was functioning properly during the release and did not detect any abnormal activity levels. The results of an isotopic analysis performed on the water prior to its transfer into the SWST were also reviewed. This analysis indicated that no activity was present in the water.

Revise Operating Procedure (OP-120.01.02) to include an additional step that requires a Chemistry Technician's signature, stating that the composite sampler has been started and/or properly adjusted for the appropriate flow rate and that Radwaste Operations has been informed of this action.

The individual responsible for the error has been counseled. This event will also be reviewed with applicable Chemistry and Radwaste personnel to stress the importance of the composite sample and to inform them of the procedure revision described above.
