ABSTRACT The plant was in Mode 5, Cold Shutdown, at 0 percent reactor power on August 9, 1988.

Technical Specifications (TS) 3.3.3.lib Table 3.3-13 Action 46 requires that flow rates from vent stacks be estimated once per four hours when the flow rate instrument is inoperable.

The Turbine Building Vent Stack Flow Rate Monitor 'was'declared inoperable at 1600 hours0.0185 days <br />0.444 hours <br />0.00265 weeks <br />6.088e-4 months <br /> on February 29, 1988, and flow rate estimates initiated. The Plant Vent Stack Flow Rate Monitor was declared inoperable at 1600 hours0.0185 days <br />0.444 hours <br />0.00265 weeks <br />6.088e-4 months <br /> on March 18, 1988, and flow rate estimates initiated.

At 0600 hours0.00694 days <br />0.167 hours <br />9.920635e-4 weeks <br />2.283e-4 months <br /> on August 9, 1988, while reviewing the previous shifts log sheets, it was discovered that the following flow rate estimates for both vent stacks were missed: 2000 hour0.0231 days <br />0.556 hours <br />0.00331 weeks <br />7.61e-4 months <br /> estimates for August 8, 1988, and the 0000 hour0 days <br />0 hours <br />0 weeks <br />0 months <br /> and 0400 hour0.00463 days <br />0.111 hours <br />6.613757e-4 weeks <br />1.522e-4 months <br /> estimates for August 9, 1988. The flow rate estimates were resumed at 0800 hours0.00926 days <br />0.222 hours <br />0.00132 weeks <br />3.044e-4 months <br /> on August 9, 1988'he cause of the event was personnel error on the part of the Balance of Plant (BOP) operator as he failed to perform the required flow rate estimates.

Corrective actions include A) the individuals involved have reviewed this event and have been counselled, B) on-shift operations personnel have been reminded of the importance of attention to detail in general and the estimated stack flow rate requirement in particular by means of a night order issued on August 17, 1988.

There were no safety consequences as a result of this event and is being reported in accordance with 10CBR50.73(a)(2i(i)(B) as a violation of Technical Specifications.

The required flow rate estimates were then resumed at 0800 hours0.00926 days <br />0.222 hours <br />0.00132 weeks <br />3.044e-4 months <br /> on August 9, 1988.

The vent stack flow rate is required to calculate release rates to the atmosphere from the plant stacks. When the flow rate monitor is inoperable a conservative estimate of the stack flow rate's performed every four hours by determining which fans exhausting to each st'ack are in service. The design flows of each in-service fan are then totaled to yield an estimated flow rate.

CAUSE The cause of the event was personnel error on the part of the Balance of Plant (BOP) operator as he failed to perform the required flow rate estimates.

ANALYSIS There were no safety consequences as a result of this event. Even though the stacks flow rate estimation was not done during this period, radiation monitors sampling from both the Plant Vent Stack and the Turbine Building Vent Stack were o'perable and showed no increase in release concentration.

Conservative values were used (i.etP design stack flow) for those fan flow estimates that were missed.

1. The individuals involved in this event have reviewed this information and have been counselled.
2. The on-shift operations personnel have been reminded of the'mportance of attention to detail in general and the estimated vent stack flow rate requirement in particular by means of a night order issued on August 17, 1988.
