The plant was in Mode 5, Cold Shutdown, at 0 percent reactor power on October 16, 1987. Technical Specifications (TS) 3.3.3.11 Table 3.3-13 Action 46 requires that flow rates from Waste Processing Building (WPB) Vent Stacks 5 and SA be estimated once per four hours when the flow rate instrument is inoperable.

On October 16, 1987 the flow rate instruments for WPB vent stacks ~ and 5A were declared inoperable. Flow rate estimates were made at 1200 hours0.0139 days <br />0.333 hours <br />0.00198 weeks <br />4.566e-4 months <br /> and at 2000 hours0.0231 days <br />0.556 hours <br />0.00331 weeks <br />7.61e-4 months <br />, a period of eight hours. Flow rate estimates were not recorded for the 1600 hour0.0185 days <br />0.444 hours <br />0.00265 weeks <br />6.088e-4 months <br /> reading as required by TS. The missed reading was discovered by Radwaste personnel during a review of the log sheets.

The cause of the event was personnel error on the part of the Radwaste Control Operator as he failed to record the 1600 hour0.0185 days <br />0.444 hours <br />0.00265 weeks <br />6.088e-4 months <br /> flow rate estimate on the log sheet.

Corrective actions include A) a meeting was held for all shift foreman to discuss this and similar events, B) the operator involved was counseled on this item, C) the stack flow rate estimates are now logged at two hour intervals, D) a copy of this LER will be routed to all Radwaste Foreman to be reviewed by each Shift Foreman, and E) problems with stack flow instruments will be resolved such that stack flow estimates are no longer required to be logged.

This event is being reported in accordance with 10CFR50.73(a)(2)(i)(B) as a violation of Technical Specifications.

DESCRIPTION:

The plant was in Mode 5, Cold Shutdown, at 0 percent reactor power on October 16, 1987. Technical Specifications (TS) 3.3.3.11 Table 3.3-13 Action 46 requires that flow rates from the Waste Processing Building (WPB)(EIIS:NE) Vent Stacks 5 and 5A (EIIS:VH) be estimated once per four hours when the flow rate instrument is inoperable.

Flow rate estimates were recorded at 1200 hours0.0139 days <br />0.333 hours <br />0.00198 weeks <br />4.566e-4 months <br /> and then again at 2000 hours0.0231 days <br />0.556 hours <br />0.00331 weeks <br />7.61e-4 months <br />, a period of eight hours. The stack flow rate is required to calculate release rate to the atmosphere from the plant stacks. An estimated flow rate, when used, provides a more conservative number when determining total dose. This is performed by determining which fans exhaust to each stack that are in service by (1) inspection of the WPB Heating, Ventilating and Air Conditioning (HVAC) Panel for fans which are started/stopped from that location, (2) verbal communications with Chemistry personnel for those fume hood exhaust fans which are startedlstopped from the hot lab. The design flows of each in-service fan are then totaled to yield an estimated flow rate.

CAUSE'he cause of the event was personnel error. The radwaste operator had obtained the required information', however, he failed to enter the 1600 hour0.0185 days <br />0.444 hours <br />0.00265 weeks <br />6.088e-4 months <br /> flow rate estimates for vent stacks 5 and 5A onto the log sheets.

ANALYSIS:

This event is being reported in accordance with 10CFR50.73(a)(2)(i)(B) as a violation of Technical Specifications.

There were no safety consequences as a result of this event. The flow rate estimates prior to and after the missed 1600 hour0.0185 days <br />0.444 hours <br />0.00265 weeks <br />6.088e-4 months <br /> readings are identical and it is reasonable to assume that no additional fans were run which would adversely affect the total release rate. Therefore, the stack flow rate was assumed to be constant throughout the interval for offsite dose calculation. ~

If additional fans had been started and then secured between 1200 and 2000, the release rate would be underestimated for that time interval.

Also, the radiation monitors for these vent stacks were operable and would have alarmed if release concentrations would have reached alarm limits.

A similar event was reported in LER-87-022-00 where an estimated flow rate was not done as required.

2) The operator involved was counseled on this problem.
3) The vent stack flow estimates are now logged at two hour intervals rather than four hour intervals as previously noted on log sheets.
4) A copy of this LER will be routed to all Radwaste Foreman to be reviewed by each Shift Foreman.
5) The vent stack flow instrument problems will be resolved such that the stack flow estimates are no longer required to be logged.
