Abstract On March 23, 1987, the radiation effluent monitor on the Turbine Building vent:

stack at the Shearon Harris Nuclear Power Plant was declared inoperable. On March 25, while the plant was in Mode 3, a Technical Specification compensatory action, a noble gas grab sample, was not taken due to an incorrect log entry. A grab sample was taken after discovery of the error and the monitor was declared operable on March 26, 1987.

Descri tion of Event On March 23, 1987, the radiation effluent monitor on the Turbine Building (TB) vent stack at the Shearon Harris Nuclear Power Plant was declared inoperable due to communication problems and water in the monitor. Technical Specification compensatory actions were implemented as required to: (1) estimate flow rates, (2) take grab samples for noble gas activity and (3) collect continuous samples for iodine and particulates.

On the morning of March 25, 1987, while the plant was in Mode 3, an entry was made in the Chemistry Log Book stating that the TB monitor was operable and the auxiliary sampling equipment needed to be removed. In fact, the TB monitor was still inoperable. A different monitor had been returned to operable status the previous day and it is believed that this led to the incorrect log entry. As a result, the Environmental'&'Chemistry (E & C) shift "turnover log on the morning of the 25th stated that the TB monitor was operable and the auxiliary sampler needed to be removed. However, the E & C turnover log also stated that a gross gas sample on the TB stack needed to be,.

taken at 1300, and the Daily Chemistry Report noted that TB stack monitor as inoperable. The day shift receiving turnover noticed the discrepancy and acted in accordance with the information in the E & C shift turnover log stating that the TB stack monitor was now operable. Therefore, the noble gas grab sample required by the Technical Specifications was not taken at 1300.

The previous sample had been taken at 0100 on March 25 and the Technical Specifications require samples every 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />. The auxiliary sampler, however, was still in place and operating continuously. All other Technical Specification compensatory actions were maintained throughout the event.

At 0100 on March 26, the E & C shift recognized that the discrepancy existed, and the monitor was still inoperable. A noble gas sample was then collected as required. The TB stack monitor was declared operable later on March 26.

Cause The cause of this event was personnel error. An incorrect log entry was made stating that the monitor was operable. In addition, the on-coming shift did not thoroughly investigate the discrepancy in the logs.

~Anal sis This event is reportable per 10CFR 50.73(a)(2)(i)(B) since a Technical Specification compensatory action was not taken (noble gas grab sample). All other Technical Specification compensatory actions were taken as required throughout the event.

The grab samples taken at 0100 on March 25 and 0100 on March 26 both showed no activity present. The average flow from the stack during the period in question was 2.62 x 10 cfm. The plant was in Mode 3. No safety systems or components were affected in this event. Thus, there were no safety consequences or implications.

Similar events are described in LERs 86-001-00,86-010"00, and 87-016-00.

Corrective Actions The event was reviewed with E & C staff and they were counseled to prevent recurrence.

2 ~ A new procedure is being developed to specifically address inoperable monitors and the approoriate E & C responses. The procedure will provide administrative control concerning the status of relevant effluent monitors and their operability.
