  • Cause was found to be improper chiller operation
  • More than one leak was found in one pin

- Flow Validation

- Enhanced Visual Inspection

Main Generator Rewind

Close-fitting slot wedges, with radial filler strips and conforming strips, are used to anchor the bar in place.

The wedges butt end-to-end in the slot. Side ripple springs are provided to suppress bar movement in the core slots.

The side ripple springs exert a sideways force on the bars, while the wedges, fillers and top ripple springs exert a radial force on the bars.

Photo 7 shows the new windings fully installed and painted.

Reactor Coolant Pump Flange Gasket Replacement

Main Flange Leakage during RF14 Main Flange Leakage on B RCP located just below CCW inlet line. Not considered pressure boundary leakage, but is RCS leakage.

Main Flange Bolts are carbon steel and susceptible to Boric Acid corrosion; all other RCP materials are stainless steel.

RCP Internal Components Te eliminate main flange leakage, pump internals must be removed and two gaskets must be replaced.

Overview

  • RF-15 Activities
  • Analysis Results
  • Refueling 16 Activities
  • Outstanding Issues

RF-15 Activities

  • Dust Sampling
  • Unqualified Materials
  • Hold-up Volume
  • Laser Scanning

Analysis Results

  • ~1400 ft2 Strainer for Each Train
  • 9.5 ft3 Fiber Load on the sump

- 2.8 ft3 TempMat

- 6.7 ft3 Latent Fiber (105 lbs latent debris)

  • High Particulate Load
  • 190 ft2 Unqualified Tape, Placards, Stickers
  • Add 30% for Chemical Effects
  • Marinite Insulation is not a problem

Analysis Results

  • ~900 ft3 RMI (Mirror Insulation) Debris
  • ~600 ft3 Transports to the Sump
  • Recirculation Sump could completely fill with RMI Debris
  • Effectively alters flow patterns in the sump reducing effectiveness of strainer surface area
  • ~500 ft3 must be stopped before the sump

CFD Model Break At S/G Cross-over nozzle in Charlie Loop Red Areas will transport Mirror Insulation Foils

Ongoing Analysis Work

  • Cumulative Effects Programs
  • Placement of RMI Debris Interceptors
  • HHSI Throttle Valves

- Analysis to determine current position

- Evaluation of Replacement

  • Sump Level for break inside the Primary Shield

Refuel 16 Modifications

  • Complete Recirculation Sump Strainer Replacement

- 2000 ft2 per train (or more)

- RHR and Spray Pump Strainers Cross-tie

  • RMI Debris Interceptors
  • Trash Racks
  • Replace Pressurizer Cubicle Door
  • Block Southern Stairwell of 436 ft
  • Remove Toe-Kick Plate on 436 ft

Preliminary Sketch For Interceptors

Outstanding Issues

  • HHSI Throttle Valves
  • Carbon/Graphite Pump Seals

Summary

  • All Modifications, Analyses and Programmatic Changes must be in place before December 31, 2007
  • GL 2004-02 response makes NRC commitments to address the issues
  • Major Modification planned for RF16
  • On going analysis of HHSI throttle valves potentially will result in RF16 modification and SI rebalance
