Status of Inspection and Enforcement Bulletins (IEBs)

l (Closed for Sequoyah Unit 2) IEB 80-16, Potential misapplication of ! a.

Rosemount, Inc., pressure Transmitters Models 1151 and 1152 with either "A" or "D" Output Codes. TVA's written response of August 29, 1980 states these instruments are not used or planned to be used at Sequoyah Unit 2.

(Closed for Sequoyah Units 1 and 2) IEB 80-19, Failure of Mercury-Wetted Matrix Relays in Reactor Protection Systems of Operating Nuclear plants designed by Combustion Engineering. TVA's written response of September 4,1980 states that this equipment is not used at Sequoyah Units 1 and ___ . v

(0 pen for Sequoyah Units 1 and 2) EIB 80-20, Failures of Westinghouse Type W-2 Control Switches With Spring Return to Neutral. TVA's written response of December 12, 1980 list the safety-related systems in which the Westinghouse W-2 switches are used. The response states that for Sequoyah Unit 1, a continuity test was completed on all the Westing-house Type W-2 switches on August 13, 1980. There were no failures found. The continuity test will be repeated every 31 days for applic-able switches listed in TVA's Special Maintenance Instruction (SMI) 0-317-14. In addition, those switches listed in SMI 0-317-14 that have neutral contacts implemented will be tested in accordance with SMI 0-317-14 after each manipulation.

TVA states that they expect to submit a report on the long-term cor-rective action by April,1981. This item is to remain open pending receipt of TVA's final response and subsequent inspection.

d.

(0 pen for Sequoyah Unit 2) IEB 80-23, Failure of Solenoid Valves Supplied by Valcor Engineering Corporation. TVA in their report dated December 30, 1980 states that their preliminary investigation indicates that the valves referenced in the Bulletin are not used by TVA.

However, TVA is awaiting information from NSSS vendors for valves used in packaged components. An updated report for Sequoyah Unit 2 will be issued by TVA before fuel load.

Licensee Identified Items (LII) ( Prior to the inspection, the licensee identified several items which were ' considered prtentially reportable under 10 CFR 50.55(e). The items are as follows: a.

(0 pen) LII 328/81-08-01 -- (TVA No. EEB 8021), Auxiliary Feedwater System Design Deficiency.

Engineering Change Notices (ECN) 2940 and 2942 with associated Work Plan Nos. S1407 and S1408 respectively, have l l been issued and partially implemented.

These work plans are to be l completed before fuel load. This item to remain open pending comple-tion of this action.

b.

(0 pen) LII 328/81-08-02 - (TVA No. EEB 8034), Offsite Power Fluctu-ations.

TVA's report of November 19, 1980 states that this item is l still being investigated and that the next report is scheduled for l release the latter part of February 1981. This information was not _ ' available at the site during the inspection. TVA has issued ECN L5096, _T L5377 and L5047 with regard to correction of this item. This item will ~ be examined at a future inspection.

(0 pen) LII 328/81-08-03 - (TVA No. 8022), Essential Raw Cooling Water (ERCW) Pump Motor Relays. Engineering Change Notice L-5320 was issued.

This item is to remain open pending a field inspection of completed work during a future inspection.

Electrical (Cables and Terminations II) - Observation of Work, Work - Activities and Review of Quality Records (Unit 2) The inspector selected the following cables for examination: 2V3213A, 2V2278B, 2V63268, 2V63278, 2V63298, 2V6346B, 2V6276A, 2V6277A, 2V6278A and 2V6279A.

. The work, work activities anc' quality records associated with routing, termination nondestructive tests, physical protection and separation, cable, cableway and/or conduit identification for the above cables were examined.

Procedures II-28, Cable Pulling; II-10, Interconnecting Cable Termination and Insulation Inspection; and II-26, Exposed Conduit and Conduit Box Inspection were used in this examination.

Within the areas examined, no violations or deviations were identified.
