Wide Range Steam Generator Level Trans-mitters, NCR EEB 8006. TVA has issued a final report dated June 26, 1980.

The subject instruments are not qualified for a post accident environment according to the TVA review required by NUREG-0588. The

. transmitters will be replaced with properly qualified transmitters.

This item will remain open pending completion of licensees corrective action and subsequent review by Region II.

Selector Switches Environmental Qualifi-

-cation, NCR EEB 8008.

On June 6, 1980 TVA informed Region II that documentation is not on file to substantiate that Cutler-Hammer type 10250T switches are environmentally qualified to withstand the

temperatures which could result from certain high energy line breaks.

TVA issued an interim report dated July 7, 1980 and a final report dated August 12, 1980.

TVA discussed in the final report that an evaluation was made of all the handswitches covered by this NCR and that the subject switches located in the pipe chase area (No. 2-HS-72-40 and No. 2-HS-72-41) will be removed before startup of Unit 2.

This item will remain open pending completion of licensees corrective action and subsequent review by Region II.

Safety Injection Pump Breaker Lockout, NCR 27P:

TVA has issued an interim report dated July 18, 1980.

During preoperational testing of the safety injection pumps it was determined that under certain conditions the 6900V breaker would lockout, preventing pump start. When this condition exists there is no visual

or audible indication to operations personnel that the breaker is not available for service. This item will remain open pending receipt of the licensees final report and review by Region II.

Environmental Qualification of Class IE Equipment, NCR's EEB 8009 through 8015 and 8019. On June 16, 1980 TVA informed Region II that documentation has not been located to establish the ability of the subject tabulation of equipment to operate in the specified ambient environment. TVA has issued an interim report dated November 21, 1980. This item will remain open pending receipt of the licensees final report and review by Region II.

LII 328/80-09-05:

Class IE Cable Qualification.

The inspector reviewed the TVA final report dated June 26, 1980, NCR EEB 8002, ECN 2802, the closed work package F716E302-006 and design drawings numbers 45N862-16, revision 17 and 19. The licensee stated in the final report that the exposed cable located inside containment between the RCS RTD's and their respective terminal boxes will be run in conduit to provide sufficient shielding and exposed cables adjacent to the containment penetrations would be wrapped with 1/16 inch lead shielding. Open work package F302IO68-001 documents the installation of conduit between the terminal boxes and tha RCS RTD's. All required work is complete with the exception of the final inspection.

The inspector observed the as-built installation for the cables from RCS RTD's terminating in terminal boxes 3001 and 3003, f.

(Closed) LII 328/79-16-03: Installation of Improper Coils in Auxiliary Feedwater and Emergency Gas Greatment System control.

The inspector

reviewed the final TVA report dated May 1,1979, NCR 7P, ECN 2338 and

drawing number 45W2614-11, revision 2 entitled,

" Wiring Diagram Auxiliary Feedwater Pump and Turbines Connection".

The inspectors observed the installation for control relays 148 R1, 173R1, 174R1, 171R1, 156R1, 172R1, 175R1, 164R1, 15-G, 25-G, 3S-G and 4S-G to assure that the correct coils were installed.

(Closed) LII 328/79-24-05: Corrosion of Cable Terminations in Junction Boxes.

The inspectors reviewed the TVA interim report dated July 24, 1979, ECN 2520 thru 22 and inspection instruction number D3, revision 2, entitled, " Junction Box Inspection".

The inspectors reviewed the licensee's program to inspect and repair corrosion of cable termina-tions in junction boxes and Foxboro transmitters.

The inspectors selected junction boxes 1110, 3652, 1752, 1751, 2304, 3293, 3290, 3289, 3288 and 3287 for inspection. The as-built installation was inspected to assure that the requirements of instruction No. D3 were specified

and accomplished.

Maldistribution of Essential Raw Cooling Water Flow to CCS Heat Exchanger.

The inspectors reviewed the final TVA report dated November 16, 1978; work plans 8653, 8653R2, 8653R4, 8653R5, 8653R6 and 8663R7; and discussed the work progress with responsible engineers. The licensee discussed in the final report that a qualified motor operator would be added to manual valve 0-67-546A

,

with electrical power supplied by emergency train 8 so as to provide the operator with the capability to close the valve from the main control room upon loss of train A power. All work is complete except for the installation of a heater circuit in tho motor operator which is being completed under another work plan.

Fire Extinguishers Not Seismically -Supported.

The inspectors reviewed TVA's final report dated December 5,1979, ECN 2731, work plan 8240 and discussed the work progress with the' responsible engineer.

The licensees final report

states in part that, " seismically qualified mounting brackets will be installed to secure all fire extinguishers located in category I

structures".

The inspectors performed a general walkthrough of plant areas and observed the as-built installation for ' fire extinguishers l

A50, A47, A59, A56, A52, C26, A24, A26, A23A, A181, A40, A129 and A118 to assure compliance with TVA committments spect fied in section E.6 of the " Fire Protection Program Revaluation", dated January 24, 1977.

The inspectors discussed the applicability of this circular for the emergency dies.
