Exit Interview The inspection scope and findi qs were summarized on July 1, 1981, with those persons indicated in paragraph 1 above. The following specific items were discussed in detail:

(0 pen) Inspector Followup Item 328/81-35-01, Inadequate test specimens used for NDE examiner qualifications.

(0 pen) Unresolved Item 328/81-35-02, Flange connections of completed safety related piping systems are not adequately bolted together.

Licensee Action on Previous Inspection Findings (Closed) Deficiency 327/80-14-01, 328/80-08-01, Failure to comply with preservice/ inservice inspection requirements.

The inspector, reviewed Revision 12 to Su veillance Instruction SI-114 titled, "Preservice Baseline Inspection for '.nnessee Valley Authority Sequoyah Nuclear Plant" and determined that it complies w'.h Section XI of the ASME Boiler and Pressure Vessel Code of 1974 with summer of 1975.

It is noted that this instruction has been revised to include only baseline inspections.

Inservice inspec-tions have been deleted from SI-114 will be included in a different instruc-tion at a later date.

It is pointed out that most of the discrepancies identified in NRC report 327/80-14 and 323/80-08 applied to inservice inspection requirements hat wer e inadequately addressed in SI-114.

Since inservice inspection requirements will be addressed in a different instruc-tion, these discrepancies will be reviewed at a later date when the inser-vice inspection instructior. is issued.

(Closed) Unresolved Item 328/80-11-01, Potential inadequate preservice/

inservice inspection program.

This item primarily addressed quality assurance as it related to the preservice/ inservice inspection program. The inspector reviewed the revised " Operational Quality Assurance Manual" N-0QAM, Part II, Section 5.1, dated February 23, 1981, and determined that it, in conjunction with SI-114 Rev 12, complies with the ASME Code and NRC requirements. This item is closed.

(Open) Deficiency 328/80-08-03, Failure of records to adequately define i

'

practical examination criteria. This item applied imarily to ultrasonic examiner qualifications, however, the licensee determined that it also applied to other NDE qualifications. The licensee has revised the practical examination criteria for magnetic particle (MT) and for liquid penetrant (PT) practical exams, but not for ultrasonic (UT) exams as yet.

The inspector considers the exam criteria for the MT and PT practicals to be satisfactory, but this item will remain open until the UT exam criteria is also revised accordingly.

(Closed) Unresolved Item 328/81-10-04, Record discrepancies on containment penetrations.

The licensee investigated the record signoffs having been made by one individual and determined that the individuals whose initials appear on the records actually performed the operations. The individual who signed their initials for them didn't realize that this practice was unsat-isfactory.

The licensee has conducted training and issued a change to

" Instruction Letter No. 7" which advises personnel not to sign someone else's signture or initials to official documents.

In addition, the lic;nsee corrected the improper date and included a statement in the preop-erational test rscord packet for the containment penetrations, explaining the reason for the entries and signoffs being in the same handwriting. This item is closd.

Unresolved Items Unresolved items are matters about which more information is equired to determine whether they are acceptable or may involve violations or devia-tions. New unresolved items identified during this inspection are discussed in paragraph 5.a.

5.

Independent Inspection Effort (Unit 2)

a.

During review of piping seismic supports, the inspector noted that four blind flange connections, adjacent to vent valves, listed below, were loose and/or incomplete.

SYSTEM FLANGE CONDITION 2-62-544 Chemical Volume Control loose & missing one stud 2-62-54S Chemical Volume Control loose 2-68-560 Reactor Coolant loose 2-68-561 Reactor Coolant loose The inspector could find no tags on the systems that would indicate work was being performed nor could the licensee's QA Section find any work requests that would provide a reason for the connections to be loose and incomplete.

It appears valve lineup had been completed and the plant is ready for fuel load.

The licensee agreed to further investigate this item and resolve accordingly. This is Unresolved Item 328/81-35-02, " Flange connections of completed safety related systems are not adequately bolted together."

b.

The inspector reviewed the practical examinations given to NDE examiner candida;es for qualification in magnetic particle and liquid penetrant examination o' welds under the preservice/ inservice inspection program.

It was noted that the test specimens used in the practical exam to verify the candidates proficiency in performing the respective exami-nation, did not represent the configuration or the surface conditions of items he is expected to examine.

Specifically the test specimen used for PT qualification is a machined smooth aluminum block.

The majority of the PT examinations that will be performed in production work are on piping or vessel welds which will be in a contour ground or as welded condition.

Thus, performing a PT on a smooth machined surface does not establish a PT examiners ability to adequately perform a PT on a weld which would present an entirely different mode of cleaning the surface, removing excess penetrant, applying developer around a curved surface and evaluating the results which would perhaps include nonrelevant indications.

The licensee agreed to review this area and consider including piping welds as part ot~ the qualification examination in both magnetic particle and liquid penetrant tests. This will be carried as Inspector Followup Item 328/81-35-01, " Inadequate test specimens used for nondestructive examiner qualification."

Inspector Followup Items (Unit 2)

(Closed) Inspector Followup Item 328/81-03-05, Amount of base metal to be penetrant examined is not specified in the PT procedure. The inspector reviewed PT procedure N-PT-1 Rev 4, which includes the requirements for base metal examination, and considers the procedure revision to be in compliance with the ASME Code. This item is closed.

(0 pen) Inspector Followup Item 328/81-10-02, Inadequate Magnetic Particle Procedure.

The inspector reviewed revision 2 to magnetic particle (MT)

j procedure N-MT-1 and found it to be satisfactory except that the amount of

'

lighting required to properly evaluate MT indications had been omitted. The licensee agreed to incorporate this requirement and indicated omission of this item was an oversight. This item will remain open until the lighting requirement is incorporated.

(0 pen) Inspector Followup Item 328/81-10-03, Inadequate liquid penetrant procedure.

The inspector reviewed revision 4 to liquid penetrant (PT)

,

procedure N-PT-1 and determined that the discrepancies identified in NRC

'

report 328/61-10 were not addressed. Furthermore, PT procedure N-PT-2 also had discrepancies that were reported, but no action had been taken to correct these discrepancies.

The licensee agreed to review these dis-crepancies and to take action accordingly. This item will remain open until appropriate corrective actions are taken.

Licensee Identified Items (50.55(e)) (Unit 2)

(Closed) Licensee Identified Item 328/81-02-33, Urconservative loads on pipe support design modifications - CEB8039.

Licensee's final report is dated April 1,1981. The inspector sighted all of the modifications made to eight pipe supports and verified they were made per the approved drawing. The eight modifications sighted are described on drawing sheet numbers as follows:

their " Program Requirements Manual" required a review and update every 6 l

months. The six month reviews had not been performed since the manual was isued in November,1977. This item will be examined further by Region II in subsequent inspections.
