Inspector concerns regarding IEB 79-02 inspection for oversized bolt holes and IEB 79-14 QA coverage were discussed with the licensee.

Other concerns regarding compli-ance with IEB 79-02 and 79-14 requirements were also discussed with the licensee.

Unresolved Items Unresolved items were not identified during this inspectio Pipe Support Base Plate Designs Using Concrete Expansion Anchor Bolts, Procedures, Work Activities and Records (Units 3 and 4)

In response to IE Bulletin 79-02, the licensee is performing an inspection of safety-related pipe support base plates that use concrete expansion anchors.

An inspection was performed to verify licensee compliance with IEB 79-02 requirements and licensee commitments.

Florida Power and Light (FPSL) construction QC Procedure QI 10.24 Rev. 1-Inspection and Testing of Concrete Expansion Anchors for Installed Pipe Supports was reviewed.

Work activities in Unit 3 containment was observed.

Walkdown and pipe support identification of supports shown on sketch FSK-H-443 was observed.

It was determined that this was a reinspection because the original inspector had mismarked the support numbers and the inspection records.

It was further determined that the inspector who performed the original inspection had just been trained and that this was his first inspection.

He had not been accompanied by experienced inspectors on this first inspection.

Upon discussing the problem with the licensee, the licensee committed to include an actual inspection with an experienced inspector as part of the training of new inspectors.

A visual inspection of Unit 3 support 249A-B on sketch E-2387-1C249 was performed.

The baseplate for support 249A-B had enlarged bolt holes.

A review of the inspection records for the support revealed that the oversized bolt holes had not been reported.

Bechtel site engineer indicated that an inspection of the baseplate to verify correct details (including bolt hole size)

was required by the procedures.

QC personnel, however, indicated that a specific inspection verification of bolt hole oversizing was not required of the inspectors.

Upon being appraised of IE Bulletin 79-02 requirements, the licensee committed to perform a sampling reinspection of baseplates for bolt hole oversizing in accordance with IE Bulletin 79-02 requirements.

Pending completion of licensee commitments, and resolution of the above noted inspector concerns, IE Bulletin 79-02 shall remain open.

No items of noncompliance or deviations were identified.

Seismic Analysis for As-Built Safety-Related Piping - Procedures, Work Activities and Records (Units 3 and 4)

In response to IE Bulletin 79-14 the licensee is performing an inspection of safety-related piping systems to verify the seismic analysis input.

An inspection of the licensee's activities was performed to verify licensee compliance with IEB 79-14 and commitments.

FPGL document number 5177-102-B-001 Rev. 0, IEB 79-14 implementation proce-dure, was reviewed.

The licensee was informed that the procedure lacked a

verification of piping system material and a verification that correct weights for the installed valves and operators are used on the seismic analysi Unit 3 piping shown on isometric Drawing E-2387-IC-301 (safety injection)

inside the containment was reinspected.

It was noted that valve 3-875B was verified to be the correct valve but insulation had not been removed from the valve and no identification markings were visibile above the insulation.

Further inspection however indicated that the valve was not required to be verified since it was not part of the 10/ sample of which insulation would be removed.

During the inspection it was noted that QA coverage was going to be provided only for the repair activities but not for the inspection of piping systems.

The licensee was informed that this would be inadequate QA coverage.

The licensee agreed to consider providing QA coverage for the inspection acti-vities.

During the inspection it was noted that current site or corporate QA procedures do not adequately address how future modifications would be provided for on the seismic analysis.

Pending resolution of the above noted concerns and completion of IEB 79-14 requirements, the bulletin will be left open.
