Independent Inspection Effort a.

Transportation of New Steam Generator Lower Assemblies The inspector spent approximately two shifts in the direct observation of unloading and transporting of one new steam generator lower assembly

identified as Ij2993.

The following areas were checked out to ensure they complied with FPSL process sheet, PS No.79-003 Rev 1:

(1)

Rigging of the steam generator lower assembly onto the transporting trailer.

(2)

Transporting of the steam generator lower assembly over the route specified to the laydown areas.

(3)

Precautions taken when traveling over numerous safety related buried facilities.

Rigging of the steam generator lower assembly for remov'd'1 from trailer onto storage saddles.

(5)

Unloading the steam generator lower assembly onto the storage saddles at the laydown areas.

No items of noncompliance or deviation were identified in the areas inspected.

The inspector examined the following FPSL process sheets for content and completeness:

Steam Generator Shipping and Storage Management The attached Figure 1 to process sheet PS No.79-012, Rev.

1, entitled

"Steam Generator Iower Assembly Receipt, Handling and Storage" shows the steam generator shipping and storage arrangement.

It was noted during this inspection that the existing arrangements were not as specified in Figure 1.

Steam generators

$j2991 and Ij2992 in the storage laydown area and fj2993 in the transporting barge had a nitrogen protec-tive blanket in lieu of a nitrogen purge arrangement as specified in PS No.79-012, Rev.

1.

FPSL shall determine which protective method will be used on the stored steam generator and change the appropriate procedures to reflect this condition.

This is identified as Unresolved Items Nos. 250/79-25-01 and 251/79-25-01

"Steam generator shipping and storage arrangement".

Construction Status The three received steam generator lower assemblies have been removed from the transporting barge to their temporary laydown area near Unit 4 facility.

A mockup is available onsite depicting the proposed steam generator repair program.

At present, additional drawings and procedures are being prepared for the upcoming steam generator repair project.

Nitrogen Gage Calibration I

'

glir, II A detail inspection was performed on the nitrogen purge blanket pressure gages tied into the steam generator lower assemblies.

This inspection revealed that the nitrogen gage tied into the primary side of Unit jj2991 had no calibration sticker.

The Westinghouse calibration records for this gage were reviewed and determined to be satisfactory.

In addition, FPM immediately had this gage put into their calibration system and tests revealed that the gage was within calibration. It was also noted by FPM quality assurance department that the calibration stickers were checked prior to transporting from the Westinghouse plant in Tampa, Florida.

All indications are that the calibration stickers had apparently been blown off or tora off during transport.

No items of noncompliance or deviation were identified in the area inspected.

Overexpanded Tubes Review of Westinghouse Deviation Notices DN jI018961 and jI018972 revealed that numerous tubes on units jj2991 and jI2993 were overexpanded during hydraulic expansion operations.

Further investigation reveals that units jj2991, jI2992, and jI2993 have had several tubes plugged during construction of the steam generator lower assemblies.

The inspector requested that FPRL furnish him with engineering evalua-tions regarding the acceptability of the above conditions.

This is identified as Unresolved Item 250/79-25-02 and 251/79-25-02 "Overexpanded Tubes".

No items of noncompliance or deviation were identified in the area inspected.
