AECM-83/0243, 4/15/83 AECM-83/0326, 6/13/83 On April 15, 1983, Mississippi Power & Light Company notified Ms. L.

Watson, of your of fice, of a Reportable Deficiency at the Gr .ad Gulf Nuclear Station (CGNS) . The deficiency concerns the inability to maintain a pressurized water supply to the RHR Pumps and discharge piping.

MP&L has evaluated this deficiency and has determined that it is reportable under the provisions of 10CFR21 for Unit 1, and 10CFR50.55(e) for Unit 2.

Details are provided in our attached Final Report, which was originally due on February 1,1984, but an extension until March 2,1984, was granted by Mr. Paul Fredrickson.

A. Description of the Deficiency The. deficiency concerns 'the f ailure of the A/E to provide a pressurized water' supply that would ensure that the Residual Heal Removal (RHR) pumps', A and B, and discharge piping are filled continuously with water as' required by the C. E. design specification.

The purpose of the pressurized water supply is to avoid delays in fill-

'ing the RM pump discharge piping and to avoid water hanmer ef fects during RL:( pump starts.

The RHR jockey- pump subsystems, designed by the A/E, were to provide the necessary pressurized water supply to the RHR pump and discharge piping.

Howe.ver, the jockey pump logic design caused the jockey pump to trip

- when it was required for_ RHR system pressurization.

~ The ~ jockey pump logic' was designed so that either the jockey pump min-flow valve or the Feedwater Leakage' Control (FWLC) system supply valve must;be open to allow jockey pump operation. - .With both the min-flow.

valve and the' FWLC supply valve cicsed the jockey pump would trip, removing the pressurized source of water from the RHR pumps and dis-charge piping. MP&L has determined that this design feature was incorporated ,to prevent dead-head' operation of the jockey pump.

.However, this violated the G. E criteria since there would be no aasurance that the RHR discharge line would remain in a filled condition without 'a' jockey pump running. -

B.- Analysis of Safety Implications During normal operations t'he jockey pumps supply to the FWLC system is closed and the jockey pumps min-flow valves are open. This allows the jockey pump to supply press'urized water to the RHR pumps and discharge

_ piping. A containment isolation signal (high drywell pressure of RPV Level 2) will close the jockey pump min-flow valve, stopping the jockey pumps.

'During a LOCA, as Reactor Pressure Vessel (RPV) water level is

' decreasing, the jockey pumps will trip at a RPV Level 2. However, the RHR pumps would not automatically start until RPV water level reached Level 1. During this period of time the RHR discharge piping could become depressurized and partially drained. This would create a water hasser, during the RHR pump start,. which could result in damage to the RHR piping, valves, and piping supports.

MP&L has determined that this deficiency could create a substantial safety hazard.

Mississippi Power and Light' received information of the deficiency on March 15, 1983. We reported the deficiency to Ms. L. Watson, of your office, as being reportable ' unde'r the provisions of.10CFR21 for Unit 1 on April'15, L1983. An evaluation for Part 21 has been completed and the MP&L " Responsible

The design deficiency of the' RH1 jockey pump logic is located at Grand C,21f

. . Nuclear Station,= Unit s 1,and 2. .Our A/E has issued QAR-F-395 to provide a

< tracking mechanism for Unit 2 and has initiated action to incorporate the Unit

~1" design modifications, as appropriate. This . is a plant specific design and

'1we have 'no knowledge of such deficiencies ~ other than at GGNS. .

A. ~ Corrective Actions Taken MP&L has completed ~ hardware modifications per 's Design Change Package

(DCP-83/5006) to correct the subject deficiency. on Unit 1. This-

. modification.will prevent the automatic tripping .of the jockey pump due y  : to the valve closures. In addition to the removal of the pump trip, the automatic containment isolation: signal was removed from the valve, in the min-flow line to- prevent pump dead-heading. To provide containment

isolation, the valve in the min-flow line will automatically isolate any time the jockey pump is not running. The LOCA automatic containment isolation signal is not needed because this is a nuclear safety-related system and the jockey pump discharge pressure is higher than the containment design pressure.

DCP 83/5006 hardware changes were completed prior to nuclear heatup.

This design change was reviewed by the NRC as it required a GCNS

. Technical Specification change.

The A/E hse initiated actions to incorporate the Unit 1 modifications, as appropriate, on Unit 2. This action will be tracked on QAR-F-395 for
