On July 17, 1978, Mississippi Power 6 Light Company notified Mr. V. L.

Brownlee, of your office, of a Potentially keportable Deficiency (PRD) at the Grand Gulf Nuclear Station (GGNS) construction site. The deficiency concerns GE type CR2940 switches that were insthlled without locking rings.

This deficiency was determined to be reportable under the provisions of 10CFR50.55(e) as reported in our letter of August 15, 1978. As a result of a review of previously reported PRD's to determine the applice.bility of 10CFR21, MP&L has determined that this deficiency is also reportable under the provisions of 10CFR21. We are therefore, supplementing the Final Report submitted in our Reference 1 letter.

All details are included in our attched Supplemental Final Report.

4. Nature of the deficiency ... and the safety hazard which ... could be created by such a deficiency ...:

A. Description of the Deficiency The GE-CR2940 control switch assembly requires a locking ring which secures the switch in place on the benchboard face. An error in manufacturing documentation utilized to manufacture control room panels was responsible for the GE-CR2940 control switch being installed without the locking ring. As a result, the entire switch body may rotate rather than the operator portion of the switch, thereby preventing the switch from functioning.

B. Analysis of Safety implications GE-CR2940 control switches are utilized for safety-related manual control functions. The omission of the locking ring from the switches could delay manual operation during a plant event such that technical specification limits could be exceeded. The panels that contained CR2940 handswitches in safety-related applications are 11113-P879, 11113-P871, and 11113-P872. However, the only safety function on these panels requiring operator action is initiation of hydrogen recombiners following an accident. The plant staff would have adequate time to make associated control switches operable before safety limits were exceeded.

5. The date on which the information of such deficiency ... was obtained.

Mississippi Power and Light received information of the deficiency on July 17, 1978. We reported the deficiency to Mr. V. L. Brownlee, of your of fice as a Potentially Reportable Deficiency on that date. Since that date MP&L has filed one (1) Report to inform the Commission of the progress and status of the evaluation of this deficiency. An evaluation for Part 21 has been completed.

6. In the case of the basic component ... the number and location oc all such components.

The subject switches are located in panels 1H13-P870,1H13-P871, and 1H13-P872 in Unit 1 at Grand Gulf Nuclear Station. Manufacturing documentation has been revised to assure correct installation for Unit 2.

Locations of other defective switches were reported by General Electric in their letter of July 19, 1978, to NRC Region V.

7. The corrective action which has been taken ... the name of the individual

taken to complete the action.

A. Corrective Actions Taken General Electric issued Field Disposition Instruction (FDI)-WAEG on August 1, 1978, to correct the deficiency on Unit 1 panels. All defective CR2940 switches were removed and reinstalled with locking rings.

The switches had not yet been installed on Unit 2. The manufacturing documentation has been revised by General Electric to assure correct installation with the locking rings.

C. Length of Tine to Complete Actions MP&L received notification of the deficiency on July 17, 1978. All corrective actions had been completed by March 6, 1979.

As the deficiency did not originate with MP&L, we have no advice to offer.
