On Septe=ber 19, 1980, Mississippi Power & Light Co=pany noti-fled Mr. M. Hunt of your office of a Potentially Reportabic Defi-ciency (PRD) at the Grand Gulf Nuclear Station (CGNS) construction site. The deficiency concerns instrument tubing failu.re on the Standby Diesel Generator.

Our investigation as to reportability of this tsatter under 10CFR50.55(e) and 10CFIL.I is continuing. Our status report is at-tached. We expect to file our final report on this tsatter on or before March 5, 1981.

1. Descripticn . _ _

A flat on the outer diar.eter and running the entire length of two sections of 3/8" stainless steel instrument tubing apparently caused Icaks to occur during routine pressure testing on system P75, Standby Diesel Generator. The two sections of tubing, M1 and H2, were in panel IH22P400. In both cases, the leakage oc-curred where the piping was joined to the panel by a swedge" lock fi t t ing.

Subsequently, an inspection of all tubing used in panels 1E22P400 and lHP22P401 revea3ed one additional 4-foot section of piping in the 401 panel with a s1=ilar flat. This section, however, successfully passed the pressure test.

All of the purchased tubing (5400'fe d)'~v~ith the same heat code as the defective tubing has been'used at the GGNS construction

, site. This tubing size and material is qualified for use in i either Balance of Plant (30P) or safety syste=s. This deficiency applies to both Unit 1 and Unit 2. At this time, the spec,1fic locations of where this tubing has been used has not been. deter-mined.

II. P_r_cposed Resolution Mississippi Power 6 Light Company is working with the constructor to resolve this de.ficiency. The evaluation to determine repor-tability under 10CFR50.55(e) and 10CFR21 is in progress. After the extent of the deficienc'y has been determined, remedial action

.and action to preclude recurrence vill proceed. ~~

III. Status of Proposed Resolution The censtructor has requested engineering data from the tubine manufacturer, Sandvik, that vill be used in evaluating this 4- _ .

ficiency. Additionally, the following findings were obtained from a netallurgical analysis of the damag'ed tubing:

1. A metallographic examination (up to 200X)o 'f the tubing failed to reveal any cracks or other defects. . _ _
2. The flattened area was ceasured to be 0.0025" deep. Other scrapes on the outside surface were 0.0005" deep.

This preliminary analysis concluded that the flattened area was most likely caused by a defective die scraping on the tubing,, .

during manufacturing.

Finally, our investigation of the extent of this deficiency is continuing.
