Inoperable Containment Isolation Valves 1WL99 and lWLlOB CONDITIONS PRIOR TO OCCURRENCE:
Mode 3, Surveillance Testing in progress.
DESCRIPTION OF OCCURRENCE:
While performing surveillance during Mode 3, 1WL99 (RC Drain Tank -
Vent Header Connection) and 1WL108 (RC Drain Tank ~ N2 Connection) were de-clared inoperable because they failed to close within. the time allowed by Technical Specifications.
Action Statement for LCO 3. 6. 3-.1 was placed in effect and 1WL99 and 1WL108 were closed and isolated.
DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:
The valve operator vent solenoid was determined to be undersized.
The 3/64" ports were too small to allow adequate bleed off of air from operator.
Aging of existing solenoid combined with undersized ports, caused valve stroke time to exceed the allowed 10 seconds.
ANALYSIS OF OCCURRENCE:
Technical Specification LCO 3.6.3.1 Action Statement requires that the inoperable valves be secured in the isolation posi~ion and de-energized.
1WL99 and 1WL108 were isolated and de-energized, therefore, the LCO was not violated.
At no time was there any danger to. the health and safety of the general public or site personnel.
CORRECTIVE ACTION
Solenoid vent valves (Asco, type FT8314B6) were replac-ed with larger_
size (Asco, type FT8321A2) *valves to d_ecrease _the bleed off time.
- - 1WL99 and ___ lWLlOB were then timed and declared operable.
The Action Statement was then terminated.
