No. 12 Steam Generator Level Channel I Inoperable CONDITIONS PRIOR TO OCCURRENCE:
Reactor tripped from 100% power DESCRIPTION OF OCCURRENCE:
At 1013 hours0.0117 days <br />0.281 hours <br />0.00167 weeks <br />3.854465e-4 months <br />, No. 1 Unit Reactor was tripped from 100% power for the 100% generator trip test.
Following the trip, the No. 12 Steam Gener-ator Channel I failed to alarm on Low-Low Steam Generator Level.* Upon investigation, it was discovered that the level sensor was out of calibration.
The channel was declared inoperable at 1825 hours0.0211 days <br />0.507 hours <br />0.00302 weeks <br />6.944125e-4 months <br /> and the channel was placed in the tripped condition.
Action Statement No. 7 for LCO 3.3.l.l and Action Statement No. 14 for LCO 3.3.2.l were imple-mented.
The sensor was recalibrated and the channel calibration check completed.
No. 12 Steam Generator Level Channel I was declared operable at 1932 hours0.0224 days <br />0.537 hours <br />0.00319 weeks <br />7.35126e-4 months <br /> and Action Statement No. 7 for LCO 3.3.1.1 and Action Statement No. 14 for LCO 3.3.2.l were terminated.
DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:
The level sensor was given an initial rack calibration on 8/20/76.
The scheduled calibrations are performed every 540 days.
However, the level sensor drifted out of calibration prior to its scheduled recalibration.
ANALYSIS OF OCCURRENCE:
Action Statement No. 7 for LCO 3.3.1.1 and Action Statement No. 14 for LCO 3.3.2.1 state that with the number of operable channels one less than the total number of channels, startup and/or power operation may proceed until the performance of the next required channel functional test provided the inoperable channel is placed in the tripped condition within 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br />.
No. 12 Steam Generator Level Channel I was placed in the tripped condition within 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br /> and remained in that condition until it was returned to service at 1932 hours0.0224 days <br />0.537 hours <br />0.00319 weeks <br />7.35126e-4 months <br /> on 6/26/77, therefore, the LCO's were not violated.
