Missed Technical Specification Surveillance CONDITIONS PRIOR TO OCCURRENCE:
Mode 5 Operation, for scheduled outage 77~19.
DESCRIPTION OF OCCURRENCE:
With the unit in Mode 5 operation for outage 77-19, it was discovered that Surveillance Test 4.3.2.1.l(E) for the Train B logic was missed for January.
DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:
This problem was caused by personnel error in that the Inspection Order Request Form.for Train B logic testing was never entered into the computer.
ANALYSIS OF OCCURRENCE:
Surveillance Procedure 4.3.2.1.l(E) verifies operability of the Train B SSPS Slave Relays.
Because this procedure was missed, operability cannot be proven.
CORRECTIVE ACTION
Upon ;identification of the missed surveillance, the Train B Slave Relays were tested in accordance with Surveillance Procedure SP(0)4.3.2.l.l(E).
The surveillance verified operability.
The inspection order *system was revised to include the overlooked test.
Further action is being taken to audit the Operating Department Surveillance files to insure that no similar situations exist.
This action will be completed by 4/7/77.
