Failure of No. 12 Steam Generator Steam Pressure Channels I and II CONDITIONS PRIOR TO OCCURRENCE:

Operational Mode 1, Reactor Power: 50%

DESCRIPTION OF OCCURRENCE:

At 1755 hours0.0203 days <br />0.488 hours <br />0.0029 weeks <br />6.677775e-4 months <br />, during a severe electrical storm, No. 12 Steam Generator Steam Pressure Channels I and II indicated high.

Instrumentation per-sonnel were notified and at 1930, declared the two channels inoperable.

Technical Specification 3.3.2.l, Table 3.3-3, requires a minimum of two (2) steam pressure channels per steam line to be operable during Mode 1 operation.

With No. 12 Steam Generator Steam Pressure Channels I and II inoperable, conditions in excess of LCO 3.3.2.lb existed; there-fore, at 1930, Technical Specification 3.0.3 was implemented and a power reduction was started to comply with the 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br />, Mode 3 require-ment of specification 3.0.3.

At 2025 hours0.0234 days <br />0.563 hours <br />0.00335 weeks <br />7.705125e-4 months <br />, the pressure transmitter for No. 12 S/G pressure channel II had been replaced and channel II was declared operable.

Action Statement 3.0.3 was terminated and Action Statement 3.3.2.lb was implemented.

After placing No. 12 S/G Pressure Channel I in the tripped condition, a power ascension from approxi-mately 15% was initiated.

At 2300 hours0.0266 days <br />0.639 hours <br />0.0038 weeks <br />8.7515e-4 months <br />, the pressure transmitter for No. 12 S/G pressure channel I had been replaced and channel I was declared operable.

Action Statement 3.3.2.lb was then terminated.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The No. 12 Steam Generator steam pressure channels failed simultaneously during a severe electrical storm.

Preliminary investigation revealed that the force motor in each transmitter had failed electrically.

The cause of this occurrence is believed to have been a lightning strike in the South Penetration area, but the investigation is incomplete at this time.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.0.3 states that in the event a Limiting Condi-tion for operation and/or associated action requirements cannot be satisfied because of circumstances in excess of those addressed in the specifications, the facility shall be placed in at least HOT STANDBY

At 1930, a power reduction to HOT STANDBY was initiated.

At 2025, No. 12 S/G Steam Pressure Channel II was re-turned to operable status and Action Statement 3.0.3 was terminated.

Channel I was placed in the tripped condition and specification 3.3.2.lb was implemented.

Unit power ascension from approximately 15% was initiated.

Technical Specification 3.0.3 required the unit to be in HOT STANDBY at 1855 hours0.0215 days <br />0.515 hours <br />0.00307 weeks <br />7.058275e-4 months <br />; therefore, the unit was operable for 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br /> and 30 minutes in violation of the Technical Specifications.

This is a licensee identified item of non-compliance.

CORRECTIVE ACTION

Two new pressure transmitters were obtained from spares, bench cali-brated, and installed in place of the failed units.

A comparison check was made with other operable channels to confirm operability.
