Auto Stop Oil Pressure Switch PD-826C Inoperable CONDITIONS PRIOR TO OCCURRENCE:

Operational Mode 1, Reactor Power 43%

DESCRIPTION OF OCCURRENCE:

At 0815 hours0.00943 days <br />0.226 hours <br />0.00135 weeks <br />3.101075e-4 months <br />, PD-826C Turbine Auto Stop Oil Pressure Switch was declared inoperable due to the contact arm being bound at the pivot point.

Action Statement No. 7 for LCO 3.3.1.1 was implemented.

At 0835 hours0.00966 days <br />0.232 hours <br />0.00138 weeks <br />3.177175e-4 months <br />, instrumentation personnel had reworked the pressure switch and operability was confirmed.

Action Statement No. 7 for LCO 3. 3. 1.1 was then terminated *.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Binding of the contact arm at the pivot point caused the pressure switch to stick so that it would not have operated.

ANALYSIS OF OCCURRENCE:

Action Statement No. 7 for LCO 3.3.1.1 allows startup and/or power operation to continue until the next required functional test provided the inoperable switch is placed in the tripped condition within 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br />.

PD-826C Auto Stop Pressure Switch was reworked and returned to operable status within the one hour allowance, therefore, no. trip signal was inserted.

Technical Specification Action Statement for LCO 3.3.1.l was not violated.

At no time was there any danger to the health and safety of the general public or site personnel.

CORRECTIVE ACTION

The Auto Stop Oil Pressure* switch was disassembled, lubricated, and reassembled.

The contact arm was then exercised.

Testing confirmed operability.
