' Pressurizer Level Channel III Instrument - Inoperable.

This report was initiated by Incident Report 82-308.

CONDITIONS PRIOR'TO OCCURRENCE: ,

Mode 1 - Rx Power 100% - Unit Load 1100 MWe.

DESCRIPTION OF OCCURRENCE:

j At 0720 hours0.00833 days <br />0.2 hours <br />0.00119 weeks <br />2.7396e-4 months <br />, September'27, 1982, during routine operation, the Control Room Operator discovered.that Pressurizer Level Channel III indication was reading 6% higher than the other i channels. Channel III was declared inoperable and Technical

, Specification Limiting Condition for Operation 3.3.1.1 Action 7 was entered. At 0729' hours, the' channel bistable was placed in the tripped condition. The Pressurizer High Pressure trip function was operable providing primary protection against in-creasing pressure transients.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Pressurizer Level Channel III level transmitter had failed previously on August 12, 1982 (see LER 82-063/03L) . The failure was assumed to be of an isolated nature. Due to the similarity of the events, however, an investigation into the failures is presently being conducted.

ANALYSIS OF OCCURRENCE:

4 The pressurizer level instrumentation is utilized in the Reactor Trip System.to insure the reactor is shutdown during a design step' load decrease. Since the redundant channels and primary trip were operable, no risk to the health or safety of the public was involved.

The occurrence constituted operation in a degraded mode per-mitted by a limiting condition for operation and is reportable in accordance with Technical Specification 6.9.1.9.b.

Limiting Condition for Operation 3.3.1.1 Action 7 requires:

With the number of operable channels one less than the total number of channels, power operation may proceed until performance of the next required channel functional test provided the inoperable channel is placed in the tripped condition within one hour.

CORRECTIVE ACTION:

As noted, the channel was placed in the tripped condition in compliance with the action statement. A reactor trip had occurred while restoring Pressurizer Level Channel III to service after the first transmitter failure. To prevent a trip in the final days of the current fuel cycle, the repair of the channel was delayed until the refueling outage. License Change Request 82-22 was submitted for a one time deferral of the channel functional test to maintain compliance with Limiting Condition for Operation Action 7. A Supplemental Report will be submitted upon completion of investigation and repair of the problem.
