Auxiliary Feedwater Storage Tank - Low Level.

This report was initiated by Incident Report 82-069.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 3 - Rx Power 0% - Unit Load 0 MWe.

DESCRIPTION OF OCCURRENCE:

On April 5, 1982, during routine operation, the Control Room Operator discovered that the Auxiliary Feedwater Storage Tank (AFST) c. Stained less than 200,000 gallons, the minimum volume required by the Technical Specifications.

At 0620 hours0.00718 days <br />0.172 hours <br />0.00103 weeks <br />2.3591e-4 months <br />, the AFST was declared inoperable, and Action' Statement 3.7.1.3.a was entered.

Later that day, af ter the AFST had been restored to an operable status, the contained volume again decreased be' low the speci-fication minimum.

At 2100 ;hourc', theiAFST was declared in-operable, and Action Statdms.nt: P.7.1:3.a was entered for a second time.

These occurrences constitused[ operation in a degraded mode in i

accordance with Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

On both occasions, the AFST-level decreased as a result of feeding j

ethe steam generatora during normal steam dump operation.

In.the l

first instance, the operator inadvertantly failed to initiate i

! Lake-up to the AFST before increasing the' steam generator levels.

I On the second occasion, Demineralized Water Valve 1DR6 failed in the closed position, preventing make-up to the AFST.

The l

valve control circuitry was not yet operational, and the, manual j

handwheek -failed, preventing operation of the valve.

CORRECTIVE ACTION

On the first occasion, the AFST was filled to obtain the specified minimum volume, and was declared operable.

At 0737 hours0.00853 days <br />0.205 hours <br />0.00122 weeks <br />2.804285e-4 months <br />, April 5, 1982, Action Statement 3.7.1.3.a was terminated.

The calibration of AFST level indicators later showed that the tank volume was actually within Technical Specification limits.

The operator involved was counseled by the Senior Shift Supervisor concerning Technical Speci-fication 3.7.1.3.

Demineralized water make-up was initiated, and the AFST volume was returned to specification.

The AFST was declared operable at 2354 hours0.0272 days <br />0.654 hours <br />0.00389 weeks <br />8.95697e-4 months <br />, May 5, 1982, and Action Statement 3.7.1.3.a was terminated.

The manual operator on Valve lDR6 was properly repaired and satisfactorily tested.

Due to past occurrences of a similar nature, the control valve and a second control room level indication witn a low level alarm, have been installed to improve system remote operation.

These changes were not fully operational at the time of the occurrences, however.
