SUBJECT:

WATER HAMER IN COMPONENT COOLING WATER SYSTEM At about 11:00 p.m. (CST) on 3/11/87, during Hot Functional Testing (HFT) with primary system temperature at 350 F, a steam / water hanner event occurred in the Component Cooling Water (CCW) lines servicing the RHR heat exchanger Trains B and C. RHR Trains B and C were being placed in service. An operator apparently overlooked a step in the procedure which led to introducing water to the primary side of the RHR heat exchangers without having CCW flow established. When CCW flow was established, extreme fluctuation of CCW surge tank level was experienced.

Loud banging associated with CCW trains B and C was heard in the Reactor Containment Building and Mechanical Auxiliary Building. The system was secured.

A quick evaluation indicated one box support, three hangers, and one section of CCW piping were badly damaged. Other minor damage was also apparent. All HFT activities have been put on hold pending an engineering and operations evaluation.

The licensee has issued a press release.

The state of Texas will be informed.

Region IV received notification of this occurrence by telecon from the SRI at 8:00 a.m. 3/12/87. Region IV has informed IE/NRR/SP/PA.
