PRELIMINARY NOTIFICATION OF EVENT OR UNUSUAL OCCURRENCE -- PNO..IV-88-30 This preliminary notification constitutes EARLY notice of events of POSSIBLE safety or public interest significance. The information is e.s initially received without verification or evaluation, and is basically all trat is known by the Region IV staff on this date.

SUBJECT:

LOSS OF CONTROL R00fi RECIRCULATION VENTILATION AT 4:10 p.m. (CDT), on April 18, 1988, with the plani. in Mode 1 at 30% power, the "A" train contreil room ventilation makeup supply damper (FCV 9584) failed to close during surveillance. The "C" train equipment, including No. 13 Emergency Diesel Generator (EDG) had been previously secured for a maintenance outage causing the "C" train control room ventilation system to be inoperable.

Technical Specification (TS) 3.8.1.1.d.1 associated with the EDG requires that the failed equipment associated with another train be repaired within 2 hours2.314815e-5 days <br />5.555556e-4 hours <br />3.306878e-6 weeks <br />7.61e-7 months <br /> or the plant be placed in hot standby within 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br />.

The licensee declared a Notification of Unusual Event (NOVE) at 8:00 p.m., as required by procedure when a plant shutdown is required by the TS. The condition was evaluated by the license and the NOVE was exited at 8:01 p.m.

(CDT).

The licensee repaired the failed damper at approximately 11:00 p.m. (CDT) and the plant was returned to power escalation (30% plateau testing) from 13% power.

The NRC resident inspector is monitoring the licensee's actions.

The licensee plans to issue a press release.

The state of Texas has been infomed.

Region IV received notification of this occurrence by phone from the HD0 at 8:20 p.m. CDT. Region IV has infomed EDO, NRR, and PA.

This information has been confimed with a licensee representative.
