SHUTDOWN GREATER THAN 72 H0VRS (OnlJune'28,,1990, the "D" steam generator power operated relief valve (PORV) was h ;found: to have congealed electro hydraulic control -(EHC) fluid. The PORY was M"

declared" inoperable.at 9:20 p.mL the same day and a 7-day LC0 was entered. The-6cause of the congealed EHC fluid was-found to be the heat and moisture.from a ibody-to-bonnet steam leak on.the PORV. This type of EHC fluid-(Fyrquel 220) is.

known to rapidly polymerize with the presence of heat, moisture,-and' copper.

-During work on:the.PORV, it was>also noted that the piston for the nitrogen n ' accumulator had-jammed which effectively removed the: nitrogen buffer and caused.

mthe EHC~ pump to rapidly cycleon and off. This condition may have'added additional heat;to assist in the polymerization of the EHC fluid. - On July 5,--

' r1990,'the licensee determined that the PORV could not be repaired prior to the-N .
