10CFR21 REPORT - C0:4TAINMENT BUILDING EMERGENCY ESCAPE AIRLOCK The attached 10CFR21 Report provides information on the apparent design deficiency discovered in the Containment Building Emergency Escape Airlock supplied by Enerfab Inc. As a result of an evaluation of the deviation, concerning an improper can design that allowed equalizing valves to be open at the wrong times, it was determined that this occurrence is reportable in accordance with 10CFR21 " Reporting of Defects and Noncompliances".

This correspondence confirms a telecon report made to W G Guldemond of your staff August 8, 1986 and thus conforms with the notification criteria of 10CFR21.21(b)(2).

The emergency escape airlock is provided with two valves to equalise  :

any differential pressure that may exist across the airlock doors.  !

The valves are actuated by a cas, such that, as the inner door is l opened, the differential pressure between the inside of the containment building and the inside of the airlock is equalised. In the same manner, as the outer door is opened, the airlock is vented to the  ;

outside of containment. At any time that one airlock door is open, i the opposite door and its equalizing valve must be securely closed. -

As a result of an improper can design, the coordination between ths airlock doors and the equalizing valves was not correct. The can profile for the outer door caused its equalising valve to lift as the inner door was opened. Similarly, the can for the inner door caused its equalising valve to lift when the outer door was opened.

The two actuating cams for the inner and outer equalizing valves have been modified in that an appropriate section of the can profiles were removed with a grinder. Proper operation of the actuating mechanism has been visually verified.
