This information,
- originally submitted to NRC by letter dated October 21, 1988, confirmed the telecon report made to W L Axelson of the Region III staff on October 18, 1988 and thus conformed with the notification criteria of 10CFR21.2l(b)(2). is being provided as an update to Consumers Power Companys' October 21, 1988 submittal and to confirm telecon reports made to B Burgess of the Region III staff on December 16, 1988 and J J Petrosino of the Division of Reactor Inspection and Safeguards on December 21, 1988; The information contained in this Attachment was derived as the result of corrective measures being taken in response to Consumers Power Companys' submittal of October 21, 1988.

MASONEILAN VALVE INTERNALS DEFICIENCY 10CFR21 REPORT The following information is provided in accordance with 10CFR21.21 when a deficiency is considered to be reportable under Part 21:

- 2) Facility Identification, activity or basic component which fails to comply or contains defects is:
Masoneilan valve internals (parts) procured from Sample - Webtrol Inc for use on the Palisades Nuclear Plant.
- 3) The firm constructing facility or supplying the basic component which failed to comply or contained de~ect is:
Sample - Webrol Inc
- 4) a. Nature of defect or failure to comply is:
Masoneilan valve internals (parts) procured from Sample - Webtrol as CQ (commercial grade) or non - Q were not genuine Masoneilan parts.
Parts were manufactured by CVS Inc and Cor-Val.
- b. The safety hazard which was or could be created is:
Parts dedicated for use in basic components may not meet Masoneilan specifications.

- 6) In the case of a basic component which contains a defect or fails to comply the number and loc~tion of all such components in use at, supplied for or being supplied for one or more facilities or activities subject to Part 21.
To date 12 purchase orders for 63 pieces have been identified which were procured from 1983 to 1988 as Masoneilan parts, however, parts were not genuine.
24 of the 63 pieces were purchased CQ and dedicated.
20 pieces (CQ and non-Q) of the original 63 had been issued to work orders.
The 6 CQ parts of these 20 may be installed in basic components.
- 7) a. The corrective action planned or taken is:
All suspect material remaining in the warehouse has been pulled and is being controlled.
Location of issued material is being identified for disposition or replacement if parts have not yet been replaced.
Notified Big Rock Point who is reviewing sample Webtrol purchases orders (3) for suspect valve parts.
Primarily all Big Rock Point parts are believed to be genuine Masoneilan parts.
Reviewing additional product lines distributed by Sample - Webtrol Inc. for applicability.
- b. The name of the individual/organization resonsible is:
John Buechler, Consumers Power is responsible for corrective action.
- c. The length of time required to complete action was or is estimated to be:
Issued material will be Qispositioned prior to start-up from our current refueling outage.
Remaining actions are estimated to take approximately 90 days.
- 8) Any advice related to the defect of failure to comp~y about the facility, activity, or basic component that has been, is being or will be given to purchasers or licensees:
Sample - Webtrol is the authorized Masoneilan distributor for 5 other plants:
Big Rock Point, D C Cook, Fermi, Davis-Besse, and Perry.

MASONEILAN VALVE INTERNALS DEFICIENCY UPDATE 10CFR21 REPORT The following information is provided in accordance with 10CFR21.21 when a deficiency is considered to be reportable under Part 21:

- 2) Facility Identification, activity or basic component which fails to comply or contains defects is:
Masoneilan valve internals (parts) procured from Masoneilans' Houston facility and H H Barnum, the previous regional Masoneilan distributor for use on the Palisades Nuclear Plant.
- 3) The firm constructing facility or supplying the basic component which failed to comply or contained defect is:
Masoneilan Inc
- 4) a. Nature of defect or failure to comply is:
Masoneilan valve internals (parts) not meeting dimensional manufact.uring tolerances.
Valve Pins:
Six of 97 pins examined were non-conforming in that they have undersized expanded diameters.
Five of these items were supplied by Masoneilans' Houston, Texas facility and the remaining item was supplied from H H Barnum, a previously authorized Masoneilan distributor.
Seat Rings:
Seven of 51 seat rings examined were non-conforming regarding manufacturing tolerances.

The two remaining, purchased in 1978, were manufactured by CVS Inc.

Parts dedicated for use in basic components may not meet Masoneilan specifications.

All physically identified non-conforming valve parts were in warehouse stock and have been removed from stores.
The quantity of parts inspected represents approximately 10 percent of existing Masoneilan warehouse stock.
- 7) The corrective action planned or taken is:
- a. Remaining Masoneilan warehouse stock will be tnspected to verify conformance with Masoneilan design tolerances.
- b. The name of the individual/organization responsible is:
John Buechler, Consumers Power is responsible for corrective action.
- c. The length of time required to complete action was or is estimated to be:
Actions are estimated to take approximately 60 days.
- 8) Any advice related to the defect of failure to comply about the facility, activity, or basic component that has been, is being, or will be given to purchasers or licensees:
None at this time.
