j To date 12 purchase orders for 63 pieces have been identified which l vere procured zrom 1983 to 1988 as Masoneflan parts, however, parts were not genuine. 24 of the 63 pieces were purchased CO and dedicated. 20

pieces (CQ and non-Q) of the original 63 had been issued to work orders. The 6 CQ parts of these 20 cay be installed in basic 4

components.-

.The purpose of this. meeting was to provide a forum for -all involved af filia-tions to presenteinfornation_regarding the procurement o t' -the' suspe_ct valve parts and ask questions'of other involved parties.

The issue regarding the suspect valve parts surfaced on.0ctober 14, 1988, when D Ellis , Masoneilan, _ identified -parts issued . for work on turbine bypass valve,.

2 t he part s distributor, Sample-Webtrol Controls, Inc (S-W) regarding the s u s te c t parts. On Oc t o be r 17, 1988, S-W had confirmed that several CPCo purchase orders f or Masoneilan replacement part s had been filled with parts manuf actured by corporations other than Masoneilan. This issue was presented to Plant Hanagement at the Plant Corrective Action Review Board (CARB) meeting a t 0830 on Oc t obe r 17, 1988.

At 0930 on Oc tober 18, 1988, in a conference call between S-W and CPCo, S-W inf ormed CPCo t hat they had identified ten purchase orders between 1983 and 1988 where part s other than genuine Masoneilan part s had been supplied to the Palisades Plant when Masoneilan parts numbers had originally been specified.

Of these ten purchase + orders, two were identified as QC with the remaining eight NQ. S-W further identified that the part s were manuf actured by either CVC, Inc , or COR-Val, both of Houma , LA.

The f ollowing represents information, statement s, and questions provided by the representatives present.

CPCo

1. Since the Oct ober 18, 1988, conference call, two additional purchase orders were identified for a total of 12 (two CQ, ten NQ). CPCo did not ask f or certificates of aut henticity for the two CQ purchase orders. A total of 142 purchase orders for Kasoneilan parts have been placed through S-W since 1977 (24 Q, 40 OQ, and 78 NQ).
2. Through the 12 purchase orders, a total of 63 parts were not manufactured by Ma soneil an. Of the 63 parts,12 have been issued to Work Orders (six CQ parts).
3. Approximately 45 percent of all Masoneilan part s procured for use at Palisades.were through S-W. All orders through S-W were procured for stock with a delivery time of approximately seven weeks.

Hasoneilan

1. Masoneilan presently does not authorize other vendors to manufacture their parts. Between September 1975 and September 1980, CVS Inc was authorized to manuf acture Haroneilan parts.
2. All nuclear _ plant orders, Q or WQ, are filled through Masonellan's Massachusetts facilities.

3. There are ways in most cases to identify genuine Hasonellan parts. Both manuf acturing tolerance and stock raterial source may af f ect component o pe ra bi l i ty.

4 Hasoneilan does not authorize the use of non-Hasoneilan parts in their i

valves; nor do they authorize their distributors to sell anything other then genuine Hasoneilan parts. .

l 4. A portion of S-W stock was bought as one lot (without all records) from i another distributorship.

1. J J Petrosino asked S-W how parts not manuf actured by Masoneilan got l

4 substituted. S-V responded thtt at a time, CVS, Inc was an authorized ~

3 manuf acturer that was utilized when lead-times were short. A S-W employee i who procured parts from CVS, Inc after 1980 has had employment te rminat ed l approximately one year ago.

i 2. J J Petrosino asked Masoneilan if part numbers were changed when manufac-turing specifications are changed. Masoneilan responded that each change

i s evaluated, but not in all cases are. numbers changed.

3. J J Petrosino recommended Palisades verif y adequacy of all Hasonellan

!_ parts in stock.

( 4. J J Petrosino questioned whether other (37 in North America) authorized l- Hasoneilan distributors may be sul-stituting parts manuf actured by companies other than Masoneilan. Masoneilati acknowledged this practice in other

! industries, but were legally bounc' to a degree with respect to actions j t hey could take.
