10CFR21 REPORT OF ALLIS CHALMERS SWITCHGEAR BREAKERS REFURBISHED BY SITEK OF MILWAUKEE The following information is provided as the 30 day written report to a 10CFR21 reportable occurrence at Palisades.

This report updates the corrective actions planned and taken by Sitek of Milwaukee and the Palisades Plant. The number of purchase orders and breakers affected is also updated. In addition, a prior 10CFR21 concerning another Palisades breaker and involving Sitek of Milwaukee is referenced.

Revisions are noted by a vertical line in the right hand margin.

The detailed information required by the NRC is attached. Sitek has stated they have not supplied any nuclear plant other than Palisades~

10CFR21 REPORT On April 15, 1992, the evaluation of improperly terminated wires on the closing relay for the 2400V switchgear breaker to P-66A, High Pressure Safety Injection Pump was determined to be 10CFR21 reportable. These *improper wire terminations were completed during a solenoid operated breaker refurbishment by Sitek of Milwaukee. Seven 2400V solenoid operated breakers from the Palisades Plant were sent to Sitek of Milwaukee for refurbishment. We believe that all seven solenoid operated breakers have the improper wire terminations.

One breaker supplies power to the safety related HPSI pump, while the other six breakers supply non-safety grade equipment. To date four breakers (including the safety related one) have been inspected. All exhibited the same problem and were subsequently reworked by Palisades maintenance personnel. The remaining three breakers will be inspected during the next refueling outage.

No operational problems due to the improper terminations have been noted on any of the affected breakers. Problems expected if the terminations came loose would be a failure of the breaker to close. At the present time all the uninspected breakers are closed.

The original procurement of these switchgear breakers was before the IEEE standards were developed. The breakers were bought as commercial items and grandfathered to class IE. The breakers were refurbished by Sitek of Milwaukee, who did not have a quality program, and Consumers Power Co. applied our QA program and 10CFR21 reportability.

The seven breakers were sent to Sitek of Milwaukee for refurbishment on two separate purchase orders. The first order was sent in September 1987 and contained six solenoid breakers. The second order was sent in July 1989 and contained one breaker, the breaker for P-66A which was the first breaker on which the improper terminations were found.

1. Name and address of individual informing the NRC.

Allis Chalmers 2400/4160 V switchgear refurbished by Sitek of Milwaukee formerly Seimens Energy and Automation P.O.Box 2387 5700 W. Hemlock Milwaukee, Wi 53223 4a. Nature of the defect.

The wires on the closing circuit relay {52X) were improperly landed.

See attached sketch.

4b. The safety hazard which could be created by such a defect.

The breaker may fail to close when required if the wires come detached from the terminals.

5. The date when information of the defect was obtained.

The improper termination was first noted on 3/20/92 and corrected on the initial breaker on 3/24/92. It was subsequently evaluated and linked to the refurbishment of the breakers on 4/15/92.

6. The number and location of all such components subject to this Part 21 Sitek of Milwaukee has stated that the only nuclear facility to whom they have supplied refurbished breakers is Palisades. Palisades has identified seven (7) solenoid operated breakers which are affected.

Stored energy breakers also refurbished by Sitek do not contain the affected closing relay and therefore are not affected by this Part 21.

All seven breakers are installed in the Palisades Plant. Four of the seven have been inspected and found to contain the improper terminations.

These terminations were corrected on those four breakers. The remaining three are installed on a non-safety related bus and are presently closed.

Palisades will inspect those breakers at the next refueling outage which is the next opportunity when they will be available for maintenance.

Palisades has submitted a prior 10CFR21 report dated November 9, 1989 on a

.. Sitek of Milwaukee refurbishment of a stored energy breaker sent to them in April 1987. In that occurrence, one 2400V stored energy switchgear breaker was refurbished to the wrong revision of a drawing. On that breaker control fuses were blown when the breaker was test operated.

Palisades corrected the wiring error on that breaker. That occurrence is not considered to be directly related to this 10CFR21 report.
