This will provide written notification of a defect in Rockbestos electrical cable supplied to Consumers Power Company, Palisades Nuclear Plant.
Although the defect is uniquely applicable to one component item only, and disposition of all affected material has been accomplished, we have determined that the matter is reportable under 10 CFR part 21.
The following information is supplied in accordance with the requirements of 10 CFR 21.

Identification of the basic component which contains a defect:
Special purpose 600V 90°C shielded Rockbestos product code C48-4042.
in one length to Palisades Nuclear purchase order #2003-7120-Q.
2/C #12 AWG control cable, Reel #N07360627, 2,612 feet shipped Plant under Consumers Power Company

Nature of the defect and the safety hazard which could be created:
Nature of defect:
Incorrect shield material.
The purchase order specified a #34 AWG tinned copper braided shield, whereas the item was
.supplied with a #34.AWG stainless steel braided shield.

Due to the higher electrical resistance of stainless steel compared to tinned copper) the shield of the affe~ted cable in installed lengths exceeding 200 feet might fail to provide an effective grounding path for a short circuit flowing into the cable.
The short circuit might therefore affect the operation of the involved valve control circuit resulting in improper functioning of the valve.

The date on which information of such defect was obtained:
On the basis of* our evaluation it was determined on November 6, 1989 that a defect as defined by 10 CFR 21 existed.
This determination was preceeded.by discussions with Consumers Power _Company personnel on October 24, 1989 and November 2, 1989.

The number and location of all affected components:
This component is a unique speci~l purpose construction manufactured only for Consumers Power Company.
The total quantity of*affected material is 2,612 feet.
The affected material is located at Palisades Nuclear Plant, Covert, Michigan.
- 7.
The corrective action which has been, is being, or will be taken.
The name of this organ~zation responsible for the action, and_ the length of time that will be required to complete the action:
Consumers Power has identified and dispositioned the affected cable on site.
Rockbestos has shipped replacement cable.and has authorized return of the remaining defective cable.
This corrective action is complete.

- No further advice to purchasers is necessary.
All affected mateiial is located at Palisades Nuclear Plant and is under the control of Consumers Power Company.
The Rockbestos contact on. this matter at Palisades Nuclear Plant is Mr. John Buechler, Materials Management Superintendent.
